The Physician Assistant/Nurse Practitioner will plan, direct, and evaluate resident and employee health care in ambulatory health services; examine and treat chronic and acute episodic illnesses, minor injuries, accidents, and other injuries/illnesses; performs selected laboratory testing; offers referrals to physicians, write prescriptions, and perform office procedures within the scope of expertise, protocols, and available equipment. The Physician Assistant/ Nurse Practitioner will also collaborate with a physician in the care and treatment of patients. Duties/Responsibilities: Examine new patients to orient them about services, to identify needs and treatment plans Sees ambulatory patients in the health services office for laboratory testing, examination, diagnosis, and treatment of chronic and acute episodic illnesses and minor injuries. Refers to physician as needed. Writes prescriptions and performs office procedures within the scope of expertise, protocols, and available equipment. Schedules follow-up visits in office or unit as needed. Performs physical examinations (pap tests, etc.). Coordinates health promotional activities (providing printed information, routine testing, flu shots, mammograms, etc.). Performs staff employment physicals, TB testing, etc. Examines staff after on-the-job accidents or injuries. Acts as a resource person for decisions, planning, and policies affecting the health of patients and staff. Maintains outpatient records. Records statistics and conducts research as deemed necessary by the institution. Compiles reports concerning ambulatory care at facility. Collaborates with staff to identify patients' needs and to help develop plans to meet these needs. Responsible for timely completion of documentation in patient records. Location: METROHEALTH MEDICAL CENTER Biweekly Hours: 80.00 Shift: 8-5 Trauma and Emergency General Surgery Advanced Practice Provider (APP) Position Overview: The Trauma and Emergency General Surgery Advanced Practice Provider (APP) delivers direct patient care consistent with the APP’s education, experience, and certification, in collaboration with the attending Trauma and Emergency General Surgery physicians. This includes the diagnosis, management, and referral of health care issues specific to Trauma and Emergency General Surgery (EGS). The APP will complete pre- and post-surgical management and recognize options for non-surgical management of trauma, EGS, and critically ill patients. Key Responsibilities Collaborate with Trauma/EGS attending physicians, general surgery residents, and APPs to complete daily rounds on the trauma and surgical floors and in the intensive care unit (ICU). Respond to Emergency Department (ED) consults for EGS, collecting data and interpreting laboratory and imaging results to develop and document assessments and care plans. Perform bedside procedures for both surgical and non-surgical patients Work with the team to complete timely documentation and carry out care plans effectively. Partner with Trauma/EGS APPs, Trauma/EGS attendings, surgical residents, nursing, physical, occupational and speech therapies, social work and case management to develop disposition plans with a multidisciplinary team approach. Respond to trauma cases in the resuscitation bay, perform trauma survey assessments, and stabilize high-acuity patients in accordance with Advanced Trauma Life Support (ATLS) guidelines. Promote consumer, family, and community health through preventative health care actions and patient advocacy. Department Overview The Advanced Practice Provider (APP) serving as the Vascular Surgery provider is a faculty member credentialed and privileged to serve as a licensed independent practitioner and is a member of the Oregon Health & Science University (OHSU) Professional Staff practicing in accordance with the respective bylaws. Our specialized vascular surgery team focuses on treating patients with diseases of the arteries, including carotid artery disease, aortic aneurysms, peripheral artery disease, and vein problems. Our population includes patients with atherosclerotic disease, end-stage renal disease, and genetic aortopathies. As part of an academic medical center, our vascular physicians are at the forefront of diagnosing aortic, arterial, venous, and other vascular diseases. This entails a blend of up-to-date medical practices, collaborative decision-making, and rigorous follow-up care. Regardless of the complexity of their condition, every patient can expect a patient-centered and personalized care plan. Performs physical examination and orders and interprets diagnostic tests. Performs blood draws, EKG’s, dressing changes and any other nursing interventions as needed. Formulates an assessment of participant’s condition and provides a treatment plan, including writing prescription(s) and providing ongoing follow-up of condition. Provides alternative methods of therapy to participants as appropriate. Educates staff, participants and caregivers regarding healthcare issues and treatments. Manages acute and long-term medical needs of participants in collaboration with other primary care providers and oversees the care eds of participants in nursing homes, homes, Assisted Living Facilities and/or group clinic settings. Provides emotional and spiritual support to participants. Provides management of medical emergencies as needs dictate. Ensures appropriate and timely documentation of medical care in the participant’s medical record, in compliance with InnovAge Greater PACE documentation policies. Identifies documents and corrects problems in medical records.
