Participates in the overall development of Plans of Care for PACE population. Skin and Wound Care Prescribes appropriate treatment modalities, performs medication reconciliation on PACE population. Serves as a Liaison with specialists in the community. Qualifications Education: Must be a graduate of an accredited Family Nurse Practitioner or Physician Assistant program Licensure and Certifications: Nurse Practitioner or Physician Assistant License and CPR, required. National certification, required. Experience: Minimum 3 years’ experience in a previous geriatric clinical nursing role, required. Work Environment: This position will spend 80% of time standing and/or walking while pushing, pulling, lifting, and/or carrying up to 50lbs.
