The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the Grand Junction area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. This is a 100% inpatient and operative role with no outpatient responsibilities. Position Details: · Inpatient management of neurosurgical patients across ICU, PCU, and floor. · Daily rounding and management of neurosurgical consults. · First assisting in the operating room. · Participation in procedures with a high level of APP autonomy (no resident coverage). · Day, night, weekend, and holiday coverage on a rotating schedule.
