Nurse Practitioner- Palliative/Hospice Care Full Job Description Primary Duties and Responsibilities The Home Health- Nurse Practitioner is responsible for assisting in the expedient multidisciplinary care of patients for Hoag Hospital and its affiliates, coordinating seamless patient care from admission through discharge in the Home Health setting. Providing supplemental medical care to patients with a focus on quality and patient experience, the Nurse Practitioner can perform history and physical exams, diagnoses and treats medical problems, orders and interprets diagnostic and laboratory tests, performs minor procedures and facilities the discharge process. Join our team as we innovate both in clinical and practice operations! We are looking for an APP (NP or PA) to work with our Primary Care Physicians. Successful candidate would have a sound clinical base of general medicine knowledge, is energetic, adaptable, collaborative and a team player. Location: San Leandro, Hayward Responsibilities Responsibilities: Assesses, plans, implements, and evaluates care for patients. Initiates and documents appropriate interventions; evaluates and records patient/family response and prescribes therapies as indicated. Performs procedures after appropriate training. Assesses patient/family needs and provides education. Promotes wellness and recommends community resources to meet patient and family needs. Incorporates the developing knowledge of evidence-based research into clinical practice. Developing knowledge of reimbursement procedures and documentation requirements. Completes patient charge capture and medical record documentation in a timely fashion. May provide case management for patients and their families, given limited knowledge of community resources. Other duties as assigned. Qualifications Requirements: Master of Science Degree, Graduate of accredited Nurse Practitioner program or Physician Assistant. Specialty certification required. Baccalaureate in NursingCurrent CA licensure.
