Position Summary: We are looking for an Advanced Practice Nurse to work within our Float Pool. The Role requires integration of knowledge, research, and experience to promote excellence in patient care and improve outcomes. (who have not worked at BCH in the past 2 years). Key Responsibilities: Performing a complete history and physical exam and interprets findings. Diagnosing and managing common clinical problems of the identified patient population. Planning, implementing, and evaluating the treatment plan in an interdisciplinary manner. Ordering and interpreting diagnostic studies and therapeutic interventions necessary to augment the patient's physical findings and treatment plan. Communicating these findings to the health care team, patients, and families. Participating in discharge planning with the health care team. Writing orders, prescriptions, and progress notes in the patient's medical record. Creating patient discharge summaries and communicating to the primary care providers. Participating in the daily patient care rounds, interdisciplinary conferences, and teaching activities of the service. Communicating the patient's condition and plan of care to the patient and family. Working 36 hours per week with 50% day/night rotation and every third weekend. Float APRNs will not be required to work holidays.
