Scheduled Hours 40 Position Summary The Advanced Practice Provider (Critical Care) provides advanced care to critically ill patients usually in an ICU or Emergency Department or an intensive surgical area requiring strong procedural skills, critical thinking and the ability to manage high acuity patients independently under the collaboration with an intensivist. Requirements for this role include a master’s or doctoral degree as a Nurse Practitioner (Acute Care certification) or Physician Assistant with concentrated critical care experience or knowledge. Oversees the plan of care, ordering tests, medications and treatments for complex conditions. Job Description Primary Duties & Responsibilities: Provides direct patient care to include obtaining patient history, performing patient assessment via observation, interview, and examination. Orders and interprets the full spectrum of diagnostic tests and procedures, including x-ray, electrocardiogram, hemodynamic monitoring, and laboratory tests. Performs critical care ultrasound. Provides counseling and education of patients and their families/caregivers concerning preventative health, treatment options, and community resources. Formulates differential diagnoses and diagnoses. Plans, implements, and directs treatment and management of patients with acute, critical, and/or complex conditions and evaluates outcomes of care. Documents patient history, physical examination, assessment, and plans of care in patient medical record. Manages health conditions via prescriptive authority or provides referral to other healthcare providers or community resources. Performs diagnostic or therapeutic interventions consistent with certification and state regulations. Performs invasive procedures commonly utilized in the critical care setting. Bills for professional services as permitted by collaborative practice agreement. Supports 24/7 staffing model, including nights, weekends, and holidays. Schedules staff in various critical care settings, including telemedicine. Maintains a safe, comfortable, and therapeutic environment for patients/families in accordance with established clinic/hospital standards. Works in accordance with established departmental policies and procedures, objectives, and quality assurance programs as well as safety, environmental, and infection control standards. Participates in emergency and critical care situations, providing rapid assessment, stabilization, and initiation of treatment for the acutely ill or injured patients. Regularly participates in quality improvement and process improvement activities centered on the goal of providing exceptional, evidence-based patient care. Serves as clinical expert and leadership consultant to nursing. In this role, you will be responsible for conducting in-home wellness risk adjustment assessments for Medicare members and other at-risk populations. Your primary objective will be to assess the overall health and well-being of member beneficiaries to ensure accurate and comprehensive risk adjustment coding, leading to greater value-based care. Perform: Medical history and physical examinations Review of systems Vital signs collection Medication review and reconciliation Possible preventive health screenings Hemoglobin A1C Spirometry KED (Kidney Health Evaluation) DRE (Diabetic Retinal Exam) FIT Patient Education & Engagement Deliver preventive health education and counseling opportunities. Educate patients on ongoing health monitoring, medication adherence, and chronic condition management. Support informed decision-making and ensure true, informed consent. Engage in goals of care discussions when appropriate. Care Coordination & Quality Support Identify clinically relevant diagnoses for care management and risk adjustment documentation. Communicate findings used to generate post visit summaries for the primary care provider. Identify and help close quality care gaps (e.g., screenings, labs, follow-ups). Collaborate enthusiastically with the broader care team to enhance clinical quality and patient experience. Compliance & Professional Standards Adhere to HIPAA regulations and safeguard protected health information (PHI). Maintain accurate, timely, and compliant EMR documentation. Perform additional duties as assigned in alignment with organizational goals. An enthusiastic collaborator contributing to the enhancement of care delivery Providers are expected to commit a minimum of 30 hours per month Locations: Multnomah County, OR Travel Requirements Daily travel expectations remain within a 55-mile radius of the assigned home location. NP Qualifications: Must have a valid unencumbered NP License for the state you will be working in This role requires travel up to a maximum 55-mile radius originating in the assigned job posting county Preference is given to weekday schedules Preference is given to standard working hour schedules Previous in-home risk assessment experience preferred Previous 1099 (PRN) experience is preferred but not required 3 years patient care experience preferred (primary care/adult/geriatric, EMR) May be requested to obtain additional NP licensure supported by Advantmed Access to reliable transportation that will enable you to travel to members’ homes within a designated area Strong ability to work within our EMR system Ability to work independently Bilingual is a plus $3,000 One-Time Sign-On Bonus Independent contractors are eligible to receive a $3,000 sign-on bonus after completing 30 in-home health assessments. The bonus is paid in a lump sum on the next regular pay cycle following completion. If the contractor ends the agreement within the first six (6) months, the full sign-on bonus must be reimbursed. $500 Monthly Performance Bonus Contractors may earn a $500 performance bonus in any calendar month in which they complete 30 approved assessments. A “completed assessment” means the visit has been conducted, documentation is finalized, and the assessment is reviewed and cleared by the Advantmed Provider Network. Bonus is paid on the next regular pay cycle.
