The selected candidate will be responsible for providing clinical leadership and administrative oversight for our team of APCs across multiple sites, including Christiana and Wilmington. With more than 2,000 primary heart failure discharges per year, you will have every opportunity to make a significant impact on the outcomes of our heart failure population. The NP/PA will also be required to maintain on-call coverage in a rotating call schedule with the other 4 APC’s for the LVAD program (must be within a 45 minute response time of the hospital while on call). This APP will function independently and collaboratively to provide medical care for patients from 0 through 20 years. This position will require the provider to work weekend days and weekday nights. Key Responsibilities Practices collaboratively with physicians and other health care providers to provide high quality health care to children. Performs appropriate assessment of patient/family upon referral/admission and appropriate documents history and physical exam data. Performs ongoing assessment and evaluation of patient progress and revision of interdisciplinary plan of care. May use prescriptive authority if appropriately credentialed to do so. Collaborates interprofessionally as needed. Identifies patient/family need for further resources and initiates appropriate referrals. Documents of patient care and progress on patient's chart.
