JOB DESCRIPTION Job Summary Provides screening, preventive primary care and medical care services to members - primarily in non-clinical settings where members feel most comfortable, including in-home, community and nursing facilities and “pop up” clinics. Strives to ensure member progress toward desired outcomes and contributes to overarching strategy to provide quality and cost-effective member care. Essential Job Duties • Provides general medical care and care coordination to various and/or specific patient member populations – adult, women’s health, pediatric, and geriatric. • Performs comprehensive evaluations including history and physical exams for gaps in care and preventive assessments. • Addresses both chronic and acute primary care complaints, and demonstrates ability to ascertain medical urgency. • Establishes and documents reasonable medical diagnoses. • Seeks specialty consultation as appropriate. • Orders/performs pertinent diagnostic laboratory and radiology testing for the medical diagnosis or presenting symptoms; works within an environment of limited resources and therefore uses diagnostic tests judiciously and appropriately. • Understands when a member's needs are beyond their scope of knowledge and when physician oversight is needed. • Creates and implements a medical plan of care. • Schedules appointments for visits when appropriate. • Provides post-discharge coordination to reduce hospital readmission rates and emergency room utilization. • Performs face-to-face in-person visits in a variety of settings including in-home, skilled nursing facilities, and public locations. • Performs face-to-face visits via alternative modalities based on business need, leadership direction and state regulations. • Orders bulk laboratory orders to target specific member populations. • Performs alternating on-call coverage to triage any urgent lab results and pharmacy inquiries and develops appropriate plans of care. • Participates in community-based “pop up clinics” to build relationships with communities, and address gaps in health care. • Drives up to 120 miles a day on a regular basis to a variety of locations within the assigned region. Drives beyond 120 miles as part of extended mileage may be required on special project days. Special projects may include an overnight hotel stay. • Obtains and maintains cross-state license in other states besides home state based on business need. • Collaborates with fellow nurse practitioners to develop best practices to perform work duties efficiently and effectively. • Actively participates in regional meetings. • May prescribe medications and perform procedures as appropriate. • Performs timely medical records documentation in electronic medical record (EMR) computer system. • On occasion, may be required to walk flights of stairs while carrying up to 50 lbs. of equipment. • Engages in practices constituting the practice of medicine in collaboration with and under the medical direction and supervision of a licensed physician to the degree required by state laws. • Local travel required (based upon state/contractual requirements). Required Qualifications • At least 1 year of experience as a nurse practitioner, or equivalent combination of relevant education and experience. • Active and unrestricted national certification from one of the following organizations: American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC). • Current state-issued license to practice as a Family Nurse Practitioner (FNP). License must be active and unrestricted in state of practice. • Prescriber Drug Enforcement Agency (DEA) license with authority to prescribe per state qualifications. A Fred Hutch Cancer Center ARNP/PA-C is a certified physician assistant or advanced registered nurse practitioner who provides evaluation and medical management of multiple service lines of adult oncology patients at our community site at Northwest Hospital. They will actively coordinate the complex care of cancer patients undergoing both systemic anti-cancer therapy and radiation therapy. The incumbent will manage side-effects of these therapies, order appropriate laboratory and radiographic studies; prescribe medications and refer patients to other specialists as necessary. The incumbent may provide for post-treatment cancer patients follow-up visits for the medically indicated period after completion of treatment. The incumbent will also provide psychological support to patients by participating in patient and/or family teaching and refer patients to community agencies and other resources to promote optimal adjustment and functioning. The NNP’s are able to practice in an autonomous role utilizing technical procedural skills as well as essential decision making in the care of these patients. Nursing Care Support: The NNP team provide ongoing support to the nursing staff whether on the unit or through formal education opportunities. The NNP’s are key leaders in patient care projects that support both nursing care and medical management. Resuscitation: The NNP’s attend deliveries in person at each of the medical centers and via a telehealth support platform at the other level I nursery sites to provide a consistent high level of care for newborns throughout the Legacy system. Discharge Planning: The entire NICU team works to provide an integrated family care experience to enable families to care for their high-risk infants within the NICU setting and beyond. The NICU values input from all members of the team including dedicated neonatal RT’s, consultants from dietary, OT/PT, lactation, as well as the core nursing, medical and leadership staff. Qualifications Education: As required by licensure. Experience: Minimum 2 years’ experience as an NICU Staff Nurse, Level III preferred. Licensure Current applicable state RN license required. National Certification as a Neonatal Nurse Practitioner (NNP) required. Current applicable state license as a Neonatal Nurse Practitioner with Prescriptive Privileges (NNP-PP) required. Credentialed by Legacy Medical Staff for Category I and II privileges. Completion of ECMO certification course preferred. NRP certification from the American Heart Association. Neonatal Nurse Practitioner

Overview Position Overview Legacy Health offers an exciting growth opportunity to join a well-established team of collaborative and energetic Neonatal Nurse Practitioners (NNPs). This role provides the opportunity to practice across both Level III and Level IV NICU settings within the Legacy system. NNPs are integral to delivery attendance and the medical management of high-risk neonates and infants, including daily rounding on NNP service patients. In addition to clinical care, the role includes telemedicine resuscitation coverage for Legacy Level I nursery sites and opportunities to contribute to non-clinical initiatives that support the highest standards of care for NICU patients and their families. Level IV NICU – Randall Children’s Hospital The Level IV, 45-bed NICU is located within Randall Children’s Hospital, a freestanding, nine-story pediatric facility. This unit provides comprehensive care supported by a full range of pediatric subspecialties, including cardiac and neurosurgical services, genetics, and ECMO. Care is delivered within a family-integrated model, supported by a dedicated pediatric transport team. Notably, this role does not require transport responsibilities. Level III NICU – Legacy Salmon Creek Medical Center The Level III, 22-bed NICU at Legacy Salmon Creek Medical Center is a growing unit actively expanding its NNP team. Scheduling is flexible and evolving, with 12-, 16-, and 24-hour shift options available to support work-life balance and team coverage. Scheduling & Practice Model This position is based in the Portland/Vancouver area and requires dual licensure in Oregon and Washington. NNPs rotate between both NICU sites, with frequency tailored to individual preferences while ensuring 24/7 coverage, including nights, weekends, and holidays. A well-established self-scheduling model and structured holiday tracks contribute to provider satisfaction and balance.
