Our National After-Hours Call Team plays a vital role by providing compassionate telephonic care and clinical direction during evenings, nights, weekends, and holidays—ensuring that residents receive timely, high-quality support without unnecessary transfers. In this work-from-home role, you’ll deliver after-hours care virtually (primarily by phone) to aging residents across multiple states. This position offers both autonomy and purpose—you’ll be the trusted voice and clinical partner helping residents and facility staff during critical times, making an immediate impact in the lives of older adults. Provider must be comfortable managing high call volumes and performing at least 30% telehealth visits, including evaluation of acute changes, falls, and controlled substance visits. Providers must be able to manage multiple calls independently while providing care across several states. Essential Duties & Responsibilities Serve as the first line of support for residents and facility staff after-hours, providing direction and medical care over the phone. Use Curana’s telephonic platform to take and place calls, coordinating care between facilities, hospitals, and clinics. Deliver high-quality, cost-effective care to patients—addressing acute, chronic, and behavioral health needs in collaboration with physicians and specialty providers. Perform comprehensive assessments and document encounters accurately and thoroughly in the EMR, ensuring compliance with CMS requirements. Apply Curana’s clinical protocols and practice guidelines to support safe, effective treatment in place whenever possible. Participate in mandatory education and training to stay current with standards of care. Scheduling & Hours: While shift times can vary, we provide coverage to skilled nursing and senior living facilities on weeknights from 5pm- 8am local time, continuous coverage from Friday at 5pm to Monday at 8am. Holiday coverage is also provided beginning at 5pm of the end of the last business day to 8am of the resumption of business hours. Availability and Coverage expectations for this role Weeknight shifts between 5pm and 8am Every other weekend coverage for 12-hour shifts covering night shifts 6pm-6pm on both Saturday and Sunday Overnight and holidays are required for all After Hours Call Team Members, 2 holidays required per year. Holiday scheduling is completed at the beginning of the year for advanced planning Qualifications Education and Experience: Master's Degree as a Nurse Practitioner Active licensure in North Carolina and South Carolina required. Additional active licenses in Florida and Georgia are strongly preferred. Candidates must be able be willing to obtain additional state licenses as needed within a 30-day timeframe. On an Average Day You Will: (includes, but not limited to) Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record Manage health conditions and terminal illness by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating hospice care services Complete the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations Provide clinical information to the hospice medical director for interpretation of eligibility for hospice recertification Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners Assure that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols Collaborate with the interdisciplinary group to create, review and revise the patient's plan of care Fulfill the obligation of requested and/or accepted assignments Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues Complete all other duties as assigned Requirements: Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program Must have current nursing licensure in State Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience Must have accreditation from an approved certifying body for advanced practice nursing as required by the State Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics) Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order Join a Team that embraces the reverence of life!
