Deliver patient care under the direction of a licensed provider by: Obtaining patient histories Reviewing medications and treatment plans Performing physical assessments Identifying and addressing health concerns observed or reported during home visits Perform clinical interventions as appropriate and at the direction of the overseeing provider, including medication administration (PO, SL, IM, IV), wound care, point-of-care testing, phlebotomy, Foley catheter care, IV fluid administration, and other approved nursing procedures. Maintain accurate, timely, and complete patient records, ensuring documentation is updated at each patient encounter. Provide patient and caregiver education as directed by the APP-Nurse Practitioner and interdisciplinary care team. CRNAs place their own epidurals, spinals, arterial lines, central lines and some peripheral nerve blocks. The MDMC CRNA group has a priority system to determine a go-home order with early out and late shifts spread out evenly amongst the group. Call responsibility is weekends only and consists of back-up call from home on Saturday 7-5 and Sunday 7-3 once every 6-8 weeks. Job Description Your Impact: Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home. Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients. Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions. Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital. Job Requirements What You Bring: Advanced Degree: Master’s Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family. Georgia State Required Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP). Reliable Mobility: Valid driver’s license and vehicle for local field-based travel.
