The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a “critical time” transitional period in their lives. CTI Licensed Practical Nurse: CTI is designed specifically to enhance the continuity of care during transition from hospital to community care.
