This support allows our Nurse Practitioner to see fewer patients and spend more time with them. Patient Care : Evaluate and treat center patients following standards of care. Follow level of medical care and quality for patients and monitors care using available data and chart reviews. Help coordinate patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment and home health care. Be an active participant and lead with the care team through daily huddles. Help Regional Medical Director (RMD), Physician and Center Administrator in setting a tone of cooperation in the practice. Complete all medical record documentation promptly working with a quality- based coder to improve coding specificity. Follow policy and protocol defined by Clinical Leadership. Meet with RMD about quality of care, review of outcome data, procedure and records issues. Participate in potential growth opportunities for all services within the Center. Participate in the local primary care "on-call" program of CenterWell Senior Primary Care. Assure personal compliance with licensing, certification, and accrediting bodies.
