Unit: The Transitional Care Center (TCC) cares for a complex, high-acuity patient population. Patients on this unit require high ventilatory/respiratory support (trach/vents; CPAP/BiPAP) while also having other complex medical needs, resulting in interdisciplinary care team approach to their care. There is a high emphasis on patient/family education on this unit, as we are transitioning patients from ICU level care to home and families need to learn how to care for their child's complex care needs in the home setting. PRN - 36 hours minimum within a 6-week schedule. The unit offers general medical services to all types of patients requiring cardiac monitoring including cardiology, renal, endocrinology, general medicine, pulmonology, gastroenterology, and oncology. Staff on this unit undergo special training and education regarding non-invasive positive pressure ventilation (Bi-PAP, C-PAP, and A-PAP). They also provide infusion therapy, transfusions, injections, and central line care. Patient care assistants on this unit provide support with ADLs, POC glucose testing, and 12 Lead EKGs. 3 South is a dedicated stroke unit and all RNs are NIH stroke scale certified.
