Overview The Nurse Practitioner will provide comprehensive, patient-centered care to individuals with serious illnesses. Based on market needs, the role may include caring for patients in hospice, palliative care, or both. Collaborate with Hospice and Palliative Medical Directors and interdisciplinary teams to deliver comprehensive, patient-centered care. Collaborate with Primary Care Provider (PCP)/Attending Provider and specialists (if applicable) to coordinate patient Plan of Care Perform in-depth patient assessments, including physical, emotional, functional, and psychosocial evaluations. Minimal Qualifications Education Associate's degree from an accredited School of Nursing required Bachelor's degree in Nursing preferred Bachelor's degree enrollment within 12 months of start date and completion within 4 years of start date required Experience 1-2 years RN experience required Licenses and Certifications RN - Registered Nurse - State Licensure and/or Compact State Licensure in the state or jurisdiction of the facility seeking employment or Multistate Compact state or license eligible required CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required Additional unit/specialty certifications may vary by department or business unit. RNC-OB C- Electronic Fetal Mon Perinatal nurses (labor and delivery antepartum antenatal testing) must obtain certification in Electronic Fetal Monitoring (EFM) from the National Certification Corporation (NCC) within 12 months from date of hire.
