These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. Our teams perform minimally invasive vascular procedures, including procedures that support and maintain dialysis access. PURPOSE AND SCOPE: Supports the Organization mission, vision, core values and customer service philosophy. Adheres to the Organization Compliance Program, including following all regulatory and Organization policy requirements. Provides direct nursing care pre, during and post procedure, under the direction of the Facility Manager, Director of Nursing and/or Charge Registered Nurse, utilizing standard nursing techniques to assist the physician in optimum patient outcomes following facility policies and procedures. Inspects operation of all facility equipment and reports malfunctions, and maintenance needs to ensure the safe and effective operation of all vascular access equipment. Inspects operation of all facility operating systems, including temperature checks in appropriate areas and physical structure of the facility and reports any issues to the Facility Manager. Ensures a clean, safe and sanitary environment in the vascular access facility at all times. Ensures all blood spills are immediately addressed according to the organization/FMCNA Bloodborne Pathogen Control Policies. ADMINISTRATIVE: Assists with efficient utilization of medication, laboratory, inventory, supplies and equipment to achieve supply cost goals following all guidelines established in the organization formularies. May assist in the development and revision of any applicable policies and procedures related to the facility to ensure continuous process improvement. Assists with Continuous Quality Improvement (CQI) activities including monthly reports and attending meetings as required. Assists with maintaining medication supply, including inventory control, checking for expired medications, and ensuring appropriate recording of controlled substances as required by law. Completes usage logs for all medications with appropriate documentation, as well as, records of drug disposal in accordance with local, state, and federal law. Assists with maintaining facility supply inventory, including ordering and restocking of supplies as needed. Examines emergency equipment and supplies, including stocking of the crash cart on a regular basis. Assesses hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations. Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status. Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care. Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.). Provides appropriate support at time of death and perform bereavement assessment. What is Required? Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing Current State License as a Registered Nurse RN 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Valid driver’s license and insurance and reliable transportation to perform job tasks You will report to the Clinical Team Manager. The Level 2 Nurse Coordinator successfully acts in at least two both of the following roles: Transfer Role: He/she fields calls from physicians who are seeking support in coordination of care for patients in various capacities. The NC coordinates emergent, urgent or specialty treatment by applying clinical expertise and working directly with the involved care providers and network/regional hospitals. Logistics Role: This nurse uses critical thinking and collaborative decision making to act as a traffic controller to support patient flow in each network facility in collaboration with their local resources. Transfer Back Coordinator Role: The NC leverages network resources to oversee and coordinates all transfer backs and reports out at the Network Daily Pulse Check.
