This is a remote position, where you will partner with people in your community who are enrolled in the Wisconsin IRIS program – a Medicaid long-term care option for older adults and people with disabilities. People in the IRIS program who need personal care services have the choice to enroll in the IRIS Self-Directed Personal Care (IRIS SPDC) option. Responsibilities Assesses all patients assigned and establishes and implements the Plan of Care Adheres to policies and procedures addressing the scope of services outlined in both Infusion nursing services, pharmacy services and state laws and regulations Adheres to INS and IgNS standards of practice, CDC guidelines, scope and standards of practice, and code of ethics as defined by American Nurses Association Collaborates with pharmacy staff, sales partners, ordering provider and patient to develop and monitor goals of patients’ therapies Completes patient care documentation in place of service including patients’ home or AIS Manages vascular access devices, to include insertion of peripheral infusion devices and ability to draw labs via peripheral venipuncture Educates patients and caregivers Evaluates progress toward goals and makes recommendations to physicians for changes in the Plan of Care based on patients’ needs Coordinates nursing care with home health and other community agencies as indicated Participate in on-call rotation as scheduled per branch protocol Complete expense reports monthly tracking mileage and other miscellaneous incidental expenses Complete annual competencies and mandatory training within assigned due dates Supervisory Responsibility: No Qualifications Graduate of an accredited school of nursing; Bachelor of Science in Nursing preferred Valid nursing license in state(s) of practice Minimum of one (1) year of experience as a Registered Nurse Previous infusion and home health care experience preferred Extensive experience in IV therapy preferred Current CPR certification Flexible schedule with the availability to work a variety of shift models Ability to work effectively within a multidisciplinary team Effective verbal and written communication skills Valid driver’s license and auto insurance Must meet company standards for driving record as well as driver age requirement Percentage of Travel: 100%-Regular driving to patient’s homes for visits is required as part of this position **To perform this role will require constantly sitting, standing, walking, bending, reaching, crouching, stooping and typing on a keyboard with fingers. Home Health Infusion Nurse

Job Overview The primary function of the Home Health Infusion Nurse is to provide exceptional in-home IV infusion therapy, nursing care, and response management. In addition, the Home Health Infusion Nurse will administer and monitor patients in the home during treatment and provide the necessary education on treatment regimen, condition management and treatment self-administration when appropriate. Requires the ability to work independently, be detail oriented and to handle multiple tasks in a fast paced environment Responsibilities Deliver care as delegated by physician, providing accurate and clinically appropriate infusion therapy assessments and interventions for all therapies, access devices and disease processes; assessing patient for health status changes, support systems and home environment, updating the care team and documenting in clinical record as indicated. Partner with the patient, family and care team in developing and implementing plan of care with specific goals under the direction of the physician. Initiate and coordinate care conferences with the participation of all members of the care team as needed. Provide education and support to patients and caregivers that enable them to safely and effectively self-administer medications and solutions. This includes storage, aseptic technique, equipment use, side effects and potential complications. Coordinate the discharge of patients when goals are met with the physician and other disciplines to achieve an orderly transition for the patient/family. Provide patient clinical care and coordination in a variety of environments including patient's home, infusion suites and hospital. Train patients in the self-administration of treatments and how to quickly recognize and respond to symptoms of adverse physical reactions to treatments. Maintain open communication with Charge Nurse, Nursing Leadership, Staffing Coordinator and other team members regarding scheduling, patient assignments and clinical matters Coordinate with Pharmacy team, and/or Nursing Leaders to address changes in patient condition or drug management issues. Document all interactions and communication with patients, plan of treatment, assessment, treatments, patient responses/reactions to treatment, status of disease process, and training activities in a timely manner using company issued tools Required Qualifications Graduate of an accredited school of nursing 2 years recent nursing experience.
