Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. We are seeking a dedicated Afterhours RN Care Manager to provide high-quality, compassionate hospice care outside standard business hours. This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Clinical administrative responsibilities, workflow auditing, and survey readiness support are also part of this role. Essential Functions Ensure appropriate delivery of hospice services to eligible patients and families. Provide direct in-person patient care during on-call, weekend, and after-hours shifts, including assessments, admissions, routine visits, death visits, and live discharges. Conduct initial and ongoing assessments of physical, emotional, psychosocial, and spiritual needs. Admits eligible patients into the hospice program following appropriate standards of practice established by the National Hospice and Palliative Care Organization (NHPCO) Notifies appropriate physician regarding any unusual or unexpected events or problems requiring direct physician consultation. Collaborates in providing patient care. Works with other members of the interdisciplinary patient care team, implements and reinforces patient instructional needs with patient and family offering informational material and pointing out special areas of concern. Complies with policies, procedures, regulations, and standards and takes corrective action when violations occur. Orients, instructs, and educates assigned personnel. Adheres to and supports team members in exhibiting TMCH values of integrity, community, compassion, and dedication. Adheres to TMC organizational and department-specific safety, confidentiality, values, policies and standards and adheres to high standards of personal and professional conduct. Performs related duties as assigned. Minimum Qualifications EDUCATION : Graduation from a qualified, nationally-accredited nursing program. EXPERIENCE : One (1) year of nursing experience in an acute care setting. Hospice or oncology experience preferred. LICENSURE OR CERTIFICATION: Current RN licensure permitting work in the State of Arizona and Basic Life Support (BLS) certification required. For Outpatient Hospice & Pediatrics areas, valid Arizona Fingerprint Clearance Card within 6 months of hire. Must have access to a vehicle, a valid driver's license, and current insurance. Knowledge, Skills And Abilities Knowledge of direct patient care and critical care procedures and techniques, tools, and responses required to ensure optimal patient care. Knowledge of exam indicators, contraindications and patient preparation. Knowledge of the death/dying and bereavement process. Skill in communicating in a clear and concise manner with staff involved in critical care, and physicians to ensure the proper care of patients. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to complete routine reports and correspondence. Ability to listen and accurately interpret others’ communication or instructions to take appropriate action. Ability to speak and communicate effectively. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of instructions and deal with several abstract and concrete variables. Strong clinical assessment skills. Excellent communication and interpersonal skills. Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through. Proficient computer skills.
