As an RN, you will complete hospice admissions and patient visits as needed. Applicants must have great clinical and communication skills. The Weekend RN is responsible for answering calls from patients, families and customers. The RN will make nursing visits for symptom management, deaths, and admissions as needed. This RN position must also be passionate about hospice care and be a great advocate for both hospice services and Suncrest Hospice. Schedule Saturday and Sunday 8:00 am - 8:00 pm (12 hour shifts) Qualifications Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with one year of experience under license Admissions and On-call experience preferred Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership. Work closely with patients on a 1-1 level but get strong support from a patient care team. Can work independently and on your own on a day-to-day basis with activities that include: phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations. Time management skills to manage visits, documentation, and meetings (i.e.: schedule visits, attend required meetings, and respond to urgent needs by prioritizing and triaging multiple concerns) Ability to learn an electronic medical records system. We are seeking a dedicated Afterhours RN Care Manager to provide high-quality, compassionate hospice care outside standard business hours. This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Clinical administrative responsibilities, workflow auditing, and survey readiness support are also part of this role. Essential Functions Ensure appropriate delivery of hospice services to eligible patients and families.
