As the Per Diem Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluate personal and professional performance and makes necessary changes to increase productivity and quality of care delivered. Adheres to and supports the policies/procedures/goals/objectives of hospice to assure quality patient care. Perform daily assessment and coordinate the plan of care with inpatient staff for General Inpatients (GIP). Assess patient for safety risks, abuse and neglect. Assess and document pain using the appropriate pain scale and manage patient’s pain. Document changes in patient condition and effectiveness/timeliness of care/interventions.
