Job Summary The RN - MDS Coordinator will play a crucial role in ensuring accurate and timely completion of Minimum Data Set (MDS) assessments, collaborating with interdisciplinary teams to enhance resident care plans, and maintain compliance with regulatory requirements. Identifies resident acuity and needs, helping to determine specific care needs, and coordinates interdisciplinary care planning schedule and process. Core Job Responsibilities Coordinates completion of the MDS assessment process with the appropriate participation from other health care professionals according to CMS guidelines. Collaborates with the interdisciplinary team to develop individualized care plans based on MDS assessments. Schedules required MDS updates with care team, checks for completion and submits to CMS. Checks CMS reports to ensure submissions are complete. Creates care conference schedules and care plan schedules to ensure timely interdisciplinary care planning meetings, and to establish assessment reference period to assure accurate assessments. Tracks insurance dates. Schedules end of Medicare MDS when appropriate. Notifies billing, interdisciplinary care team and pharmacy of any changes. Monitors for any resident changes in condition that may change scores to increase reimbursement. Ensures completion of daily Medicare documentation and Medicare (re)certifications. Initiates and monitors appeals to Livanta. Prepares CMI (Case Mix Index) calculations and certifications for the State. Reviews and prepares QAPI reports from iQIES and CASPER. Evaluates care programs and initiates changes as necessary to ensure compliance with regulatory requirements. Communicates State and Federal regulatory revisions to facility management to maintain awareness/compliance. Performs audits, analyzes data and assists with plan of action to correct identified deficiencies. Reviews/analyzes QI/QM data for trends and indicators of negative outcome and initiates corrective action. Coordinates the processing of Medicare determination to assure timely and accurate billing of resident accounts. Consults with other departments as appropriate to collaborate in resident care, risk management and quality management activities. Provides education to staff on MDS/PRI and care planning. Assures that resident rights to fair and equitable treatment, self-determination, individuality, privacy, property, and civil rights, including the right to wage a complaint, are well established and maintained at all times. Attends and participates in workshops, seminars, etc. to keep current in MDS related best practices. Performs related duties as assigned. Education/Experience Requirements REQUIRED: Three (3) or more years of clinical experience in an acute or long term care care setting. Strong understanding of MDS processes, regulations, and reimbursement systems (RUGS and PDPM). Excellent communication and interpersonal skills. Ability to interpret financial and statistical reports. Highly proficient with MS Office, databases, medical software and the ability to learn new applications rapidly. PREFERRED: Three (3) years of experience working as a MDS/PRI assessor. Two (2) or more years of management experience in the health care field. Ability to read, write and speak in other languages. We take patients with ischemic and hemorrhagic strokes, brain tumors, aneurysms, seizures, traumatic brain injuries, neuromuscular diseases and other diagnoses involving the neurologic system. The floor beds consist of patients requiring every 4-hour monitoring. Additionally, there are 7 Epilepsy Monitoring Unit (EMU) beds embedded in the 21 neuro floor beds. These patients are seizure disorder patients admitted electively with their anti-convulsant medications on hold and hard wired to continuous EEG. This allows the physicians to identify the area of the brain where the seizures are occurring. Many of these patients become candidates for brain mapping with surgically placed grids to further pinpoint the area causing the seizures. This mapping allows the surgeon to determine if the patient would be a candidate for surgical removal of parts of the brain to stop seizure activity and still allow the patient to have functionality. The nurse-to-patient ratio for floor patients is typically 1:5. Essential Duties & Responsibilities include but not limited to: • Implements physician orders, administers medications, starts IVs, performs treatments, procedures and special tests, and document treatment as required by company policy and local/state/federal rules and regulations. • Orders, interprets, and evaluates diagnostic tests to identify and assess patients' conditions. • Assesses and evaluates patient needs for, and responses to, care rendered. • Applies sound nursing judgment in patient care management decisions. • Provides primary and emergency care for occupational and non-occupational injuries and illnesses. • Administers over-the-counter and prescription medications as ordered. • Collaborates with the nursing team to create a Plan of Care for all patients. • Directs and guides ancillary personnel and maintain standards of professional nursing. The primary role of the Infusion Nurse is to provide clinical care and education to patients receiving a wide range of parenteral medications dispensed from Optum Infusion Pharmacy (the 'Company'). The most common route of administration is intravenous; however, medications may also be given via subcutaneous infusion or injection, intramuscular injection, or intraspinal (intrathecal and epidural) infusion. Service delivery will take place in our ambulatory infusion suite or in the home environment. All infusion services and medication delivery is provided in accordance with Optum Infusion Pharmacy policies, procedures and therapy-specific protocols, and based on comprehensive orders received from a physician or licensed practitioner. The Home Infusion Nurse fills an integral role in the clinical operations of the organization, working collaboratively with the multi-disciplinary team needed to prepare, deliver and seek reimbursement for the infusion therapy that is safely and effectively administered to each patient. Requirements of the Director of Nursing: Must have a New York RN license MUST have a minimum of 3-5+ years of experience as an ADON or DON Must have recent experience as an Assistant Director of Nursing or Director of Nursing in a long-term care/skilled nursing setting Must have long-term care experience Sub-acute experience strongly preferred SigmaCare experience preferred Must be a self-starter and energetic Strong clinical background and knowledge Proven leadership and management skills Excellent verbal and written communication skills Responsibilities of the Director of Nursing: Supervises all nursing staff in the facility Development and implementation of nursing policy and procedure Overseeing the hiring and continued employment of nursing staff Ensuring there is adequate nursing staff, and that the staff's skills remain current Overseeing nursing employee conduct Being knowledgeable of incidents at the facility Assessing the health needs of each resident #7638
