Purpose of Position: Delivers patient care through the nursing process of assessment, planning, implementation and evaluation. Administers and monitors patient responses to all medications. Directs and guides patient care, teaching and activities of ancillary personnel while maintaining standards of professional nursing within the realm of Hospital Policy. Maintains and delivers the highest quality customer service standards. Description Properly utilizes the nursing process in caring for patients by maintaining relevant, accurate and current patient records as evidenced by Nursing PI activities. Provides documentation of assessment of the patient throughout the Hospital admission including physical, psychological, social dimensions and discharge plans. Summarizes assessment data and initiate plan of care with communication of pertinent data to allied patient care providers as evidenced by Nursing PI activities. Understands, assumes, and demonstrates proficiency in the role of charge nurse. Demonstrates proficiency in Computerized Nursing Informatics (information systems). Assumes role of resource person regarding patient care, orientation and teaching staff. Participates in at least four-unit staff meetings per year and attends mandatory educational programs. Participates in identification of the aspects of care for the patient, indicators used to monitor the quality and appropriateness of care, and the evaluation of that care. Adheres to safety, blood/body fluid policies and procedures. Assesses personal learning development needs. Maintains documentation of current and ongoing professional development activities. Regularly up-dates knowledge of techniques and regulations appropriate to the age of the patient served. Shows a willingness to participate on Nursing Committees. Represents hospital in the community and within the hospital by maintaining confidentiality regarding patients and hospital matters and by presenting the hospital in a manner that will best market the services provided. Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers. Performs other work-related duties and activities as assigned or requested his/her immediate supervisor. Responsibilities Complete intake call sheet on all referrals including for inpatient as appropriate for the position. Gather all necessary demographic, clinical and financial information on call sheet. Ensure information is accurate and complete as call sheet is transmitted to hospital clinical and business office staff. Requests evaluation from ESP/ED/Referrer as appropriate. Assist Intake Coordinators when requested in interpretation of lab values, and diagnostic testing within Pembroke guidelines in order to facilitate appropriate referrals to facilities. Reviews for first call acceptance of appropriate patients based on Pembroke protocols. Notify the units of pending admissions. Send completed call sheets and insurance verification/authorization to Hospital Unit, Utilization Review and Business office. Maintain accurate, up-to-date bed availability including update hospital staff of pending admissions or cancellations. Update external bedboard on regular basis. Adhere to department procedures related to waitlisted patients, transferring cases. Contact the CEO when a review is necessary or patient delayed. Observes patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents. Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and determines progress of patient. May serve as leader for group of personnel rendering nursing care to number of patients. Must maintain the confidentiality of all resident care information including protected health information.
