In addition to signing off on physician’s orders, he/she must also: demonstrate the knowledge and skills necessary to implement the patient’s treatment plan; complete the medication reconciliation forms for all applicable PHP/IOP patients at admission and discharge, conduct education groups when necessary utilizing a variety of treatment modalities; communicate with referral sources, outside community providers, and family members as needed; and participating in discharge planning and communication to the provider for the next level of care.
