Reinforce hospice recommendations made by RN Case Managers and make adjustments as needed. Collaborate with doctors, nurses, social workers, patients, families, and our hospice staff to promote exceptional end-of-life care. Be available to answer any questions and provide education as needed regarding hospice services and philosophy.
