Incorporates discharge planning into clinical pathway or plan of care to provide patients with smooth transition from hospital to home. Provides cost-effective care by appropriate utilization of time, supplies and personnel. Evaluates patient care interventions and collaborates with all team members to make changes as necessary. Initiates and/or participates in patient care conferences. Assumes the responsibility to interact on a consistent basis with the patient and family providing coordination of the plan of care for those patients whose needs do not require the expertise of a Case Manager.
