Assists physician with exams and procedures. Facilitates consultations, pre-testing, diagnostic tests or procedures, referrals and future appointments for patients. Triages patient phone calls as needed and documents relevant information in the patient's chart. As necessary, facilitates communication between patient/family and interdisciplinary team and coordinates services related to patient care. Assist patients in locating available community resources. Prepares and administers approved medications and immunizations via oral, topical, inhaled, intramuscular, subcutaneous, or intradermal from provider orders (as permitted by state regulations and credentials) Participates in patient outreach to drive engagement among eligible patients. Ensures adherence with all applicable internal and external quality and operational certifications and requirements. Conducts quality review of the Health Center. QUALIFICATIONS High school diploma required plus graduation from a formal MA, LPN or RN training program and 2+ years of experience providing clinical care, ideally in primary care. National or state-specific MA, LPN or RN certification (dependent on local regulations). Must be able to give injections, complete EKGs, and perform other office-based procedures.
