Position Summary Orlando Health is a 3,200-bed system that includes 15 wholly-owned hospitals and emergency departments; rehabilitation services, cancer institutes, heart institutes, imaging and laboratory services, wound care centers, physician offices for adults and pediatrics, skilled nursing facilities, an in-patient behavioral health facility, home healthcare services in partnership with LHC Group, and urgent care centers in partnership with CareSpot Urgent Care. Experience: One (1) year of acute care RN experience. Responsibilities Essential Functions: • Must be willing and able to work at any Orlando Health facility as determined necessary by the Corporate Staffing Office. • Assesses patient’s needs and develops/revises an individualized plan of care based on patient needs and responses. • Evaluates the patient’s progress toward attaining expected outcomes. • Respects diversity by building respectful relationships with all team members and customers. • Functions as a patient and family advocate. • Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on assigned unit. • Demonstrates knowledge of the principles of growth and development over the life span, assesses the data reflective of the patient’s status and interprets the necessary information needed to identify each patient’s requirements relative to age-specific needs. • Prescribes, delegates, coordinates, and documents patient care in a knowledgeable, skillful, and consistent manner including, but not limited to, patient assessment, education, medication administration, treatments, and patient safety. • Demonstrates competency in nursing skills and use of patient care/unit equipment as defined by unit specific requisite skills. • Prioritizes patient care in an ongoing manner in accordance with Evidence-Based Practice Standards of Care. • Practices effective problem identification and resolution. • Delegates tasks and duties to healthcare team members in accordance with the patient’s needs and the team member’s capabilities and qualifications. • Communicates appropriate information regarding patient condition or unit concerns to other health care team members. • Maintains a safe and caring environment for both patients and family. • Integrates patient and family education throughout the delivery of care. • Participates in process improvement activities as assigned. • Demonstrates awareness of legal issues, patients’ rights, and compliance with the standards of regulatory and accrediting agencies. • Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA, and other federal, state, and local standards. • Maintains compliance with all Orlando Health policies and procedures. Develop and update individualized plans of care including patient visit frequency to meet individual needs and risk identification. Coordinate care across interdisciplinary teams including consultation with physicians and nurse partitioners. Provide patient and caregiver education regarding disease process, self-care techniques, end-of-life care, medications, nutrition and dietary needs. Communicate change in condition, or signs and symptoms of decline to appropriate provider (e.g., NP, PCP, Hospice Physician). Support the continuity of care by recognizing and supporting home health, palliative and hospice referrals as appropriate and recognizing and supporting transitions in levels of care. Document timely clinical records on all patients including screening/assessment findings, physician orders, progress notes, and care plans. Maintain effective working relationships with IDG, and involve team when patient's care plan warrants team participation to achieve desired outcome. Participate in on-call and patient support activities as applicable, conducting on-call services in a clinically competent and responsive manner.
