Your Role on the Team Position Summary Provides care coordination for transitions across the continuum of inpatient, outpatient, surgical and invasive procedural settings. Assists with collection of data necessary to support the patient safety and performance improvement plan and progress toward established performance measures. Works as a clinician to support high quality, timely care delivery. Acts a resource across the Medical Center relative to cardiac and vascular surgical patient care. Required Qualifications What It Takes to be Successful Strong written, oral communication, as well as effective problem solving and negotiation skills Resourcefulness to identify and search for information that is not readily available and ability to synthesize to form recommendations Must possess the skill, knowledge and ability essential to the successful performance of assigned duties Must demonstrate customer service skills appropriate to the job Minimum 2 years’ experience in quality, patient safety, infection control, or performance improvement methodologies (includes educational experience, if applicable) Experience working with Cardiovascular surgical patients Experience coordinating care for patients with complicated medical diagnosis Excellent written and verbal communication skills in English Current California RN license Current BLS certification Computer familiarity using Word, Outlook and Excel, or similar database software All external candidates must have a Bachelor of Science in Nursing Ability to maintain a work pace appropriate to the workload Ability to establish and maintain effective working relationships across the Health System Ability to design and implement patient and family education programs Preferred Qualifications Knowledge of University and medical center organizations, policies, procedures and forms Total Rewards We offer a wealth of benefits to make working at UCI even more rewarding.
