Description FULL TIME M-F; One Weekend Rotation a month The nurse case manager coordinates the care and service of selected patient populations through the acute care episode, across the continuum. Consults and serves as a resourced professional to staff members by identifying learning needs and providing instructions to staff member when observed to be needed.
