Greater Binghamton Health Center - Community Treatment Recovery Center (CTRC) is recruiting a Registered Nurse 2 Community to work with individuals with mental illness to prepare them for discharge to a skilled nursing facility. Collaborate with the Pulse Center to support bed management, patient flow, and emergent and non‑ emergent transfers. 8. Prioritize patient condition, anticipated needs, and potential risks, and assist with bed placement. 9. Maintain situational awareness by rounding on unit(s) to assess operational needs and barriers to care. 10. Facilitate timely discharges and support efficient patient throughput. 11. Participate in and lead investigations as appropriate, including patient safety events, complaints, and operational issues, ensuring timely escalation and follow‑ up. 12. Promote a culture of quality and safety by identifying risks, reinforcing best practices, and supporting compliance with policies, procedures, and regulatory standards. 13. Build relationships with staff and patients while educating and mentoring staff to promote accountability, quality, and safe care delivery. 14. Assumes patient care assignment when needed. 15.
