Job Description Summary: An Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home, or in an alternate infusion suite, to ensure patient safety with continuity and compliance under a physician’s plan of care. Works under structured supervision of the designated supervisor. Job Description: ​ Job Responsibilities (listed in order of importance and/or time spent) Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals. Organizes and participates in the provision of direct patient care, performs treatments, administers medications, and educates patients and families/caregivers. Modifies plan of treatment in response to changing patient status or physician orders to achieve established or revised patient care goals. Assesses patient needs and physical status at each skilled visit through health data access and patient interview. Re-evaluates patient needs through physical reassessment, response to therapy, and supplemental physician orders. Obtains and clarifies physician orders for plan of treatment revisions, informs physician promptly of significant changes in patient's condition, and provides written summary to physician within supplemental order. Provides training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance. Provides effective and safe teaching using patient-centered care approach for patient and family to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals. Effectively and timely communicates with Option Care’s Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines for billing optimization. Completes all documentation legibly and applies approved abbreviations and documentation error correction practices per Option Care’s policy. Coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care. Demonstrates compliance with agency operations, Option Care’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule. Responds promptly and appropriately to patient requests. Initiates emergency procedures as necessary. Accepts accountability for own practice through ethical and professional conduct. Follows established programs and practice within policies and procedures reflective of Option Care’s mission, values, and objectives. Observes legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Option Care information including adherence to HIPAA regulations. Speaks knowledgeably about Option Care’s scope of services and effectively instructs patients about related financial obligations for care and service charges. Attends and completes required training modules, in-services, and continuing education to maintain competency and professional licensure for demonstrated knowledge regarding the care and management of patients in the home and/or alternate care settings. Effectively provides oversight and coordination of paraprofessionals in the home setting and may act as the Supervisor in the absence of the Supervisor or Nurse Manager as applicable in accordance with state and federal regulations. Participates in multidisciplinary team conferences and provides precepting, training, and mentoring to other nurses for orientation, and onboarding and supervisory activities as assigned. Participates in nursing department on-call responsibilities. Performs other related duties as directed by supervisor. If applicable: Inserts PICC/Midlines per physician order. Troubleshoots/declots obstructed central line catheters. Monitors PICC/Midline and educates on prevention of line infection and routine care. The patient’s immune cells are removed from the body, reengineered in a lab and then infused back into the patient. These new immune cells seek out and destroy the cancerous cells. The Cellular Immunotherapy Program at Moffitt Cancer Center is an innovative treatment approach that harnesses the body’s own immune system to fight cancer. At Moffitt Cancer Center, we offer several types of cellular immunotherapy, including chimeric antigen receptor (CAR) T-cell therapy, T-cell receptor (TCR) therapy, tumor-infiltrating lymphocytes (TIL) therapy, dendritic cell vaccine and we are continually investigating additional options. These novel treatments are helping to transform the landscape for many current and future cancer patients. Benefit of on-demand pay with Tapcheck Electronic charting with PCC (Point Click Care) Unit Manager RN Registered Nurse Responsible for supervision of the delivery of care to residents on assigned Unit. Assess resident needs, develop care plans, administer nursing care, evaluate nursing care, and supervise CNAs and other personnel in the delivery of nursing care. Job Functions: This facility expects their employees to promote an atmosphere of teamwork with other employees and hospitality and comfort for its residents. Therefore, the following list of duties is not all-inclusive: Attend therapy meetings. Complete assessments of residents at admission and discharge from unit. Supervise nursing personnel assigned to the Unit. Document services rendered to residents in Unit for billing purposes. Write admission notes or evaluate admission notes and chart additional essential information. Identify and prioritize primary nursing problems. Place charting profile of prompters in front of chart. Develop care plan addressing immediate nursing problems. Attend weekly meetings, working with therapy and nursing department to optimize reimbursement and make decisions regarding decertification and discharge. Make daily rounds on the unit to monitor resident care and status or residents. Implement care plans for residents on the unit in compliance with physician’s orders. Implement established nursing policies and procedures on the unit, educating nursing support staff according to facility guidelines. Notify resident or legal representative of pending discharge 24 hours in advance. On date of discharge, chart complete discharge summary, to include discharge planning and referrals. Keep complete and accurate record of sign in/out sheets for staffing hours. Assign duties to LPNs and nursing assistants working on the unit, as directed. Work closely with the Director of Nursing in resolving grievances with nursing staff, family members, and residents. Promote favorable working conditions and relationships with the administration, medical staff, consultants, other departments, residents, family members, volunteers, agencies, and any other individual or group affiliated with Unit and the facility. Handle and report disciplinary action that requires immediate attention during your tour of duty in the absence of the Director of Nursing, Assistant Director of Nursing and Administrator. Review charting on a weekly basis for accuracy and completeness. Verify that nurses’ charting correlates with what therapy is charting. Document in resident’s chart pertinent information about significant change in resident’s condition. Maintain a safe working environment, complying with infection control and body mechanic procedures. Respond to need for staffing shortages by arranging for replacement staff. Supervise and perform treatments or venipunctures, as needed. Track infection control, exposure control, and resident care trends to provide proactive improvement. Identify areas of concern and offer recommendations and suggestions to the Director of Nursing. Fill in as Charge Nurse when necessary. Audit nursing charges as instructed by Director of Nursing. Inform Central Supply of any supplies that are needed for the Unit. Attend in-service education programs in order to meet facility educational requirements. Be familiar with Standard Precautions, Exposure Control Plan, Fire Drill and Evacuation Procedures and know how to use the information. Maintain confidentiality of resident and facility records/information. Protect residents from neglect, mistreatment, and abuse. Protect the personal property of the residents of the facility. Others as directed by the supervisor or administrator. Qualifications: Registered Nurse with current state license. Current CPR Excellent technical, assessment and documentation skills. Good leadership qualities. Organized and detailed in work performance. Good communication skills with excellent self-discipline and patience. Genuine caring for and interest in elderly and disabled people in a nursing facility. Comply with the Residents’ Rights and Facility Policies and Procedures. Oncology RN - IV Therapy and Injections

Job Requirements POSITION SUMMARY To be fully engaged in providing Quality/No Harm, Customer Service and Stewardship by functioning as part of a multidisciplinary team in an outpatient setting. The Oncology Clinic RN acts as a patient advocate, educator, and care coordinator for a specific patient population across the continuum of care. PRIMARY ACCOUNTABILITES Coordinate the multidisciplinary team (physicians, advanced practice nurses and other support staff) approach to connect patients with integrated, comprehensive services, specific to their condition. Track, order completion and follow up. Monitor patient compliance with patient treatment plan and assist patients with referrals to outside resources and care coordination as needed. Serve as the central point of contact for all oncology navigated patients, including interactions with medical, nursing ancillary service providers. Provide education to patients, families, and significant others via phone or written consultation. Demonstrate efficiency with delegated orders and data entry with oncology EMR. Provide patient support and phone triage skills for the oncology population. Follows Chemotherapy dosing based on National Comprehensive Cancer Network (NCCN) or other Chemotherapy guidelines with coordination of complex chemotherapy regimens with infusion and pharmacy staff. Demonstrates effective communication with other members of the healthcare time, promoting effective team functioning. Assist with daily telephone triage for oncology patients. Maintain clinical competency as it pertains to the clinic duties assigned. Adheres to medication management and clinical procedures. Work Experience MINIMUM QUALIFICATIONS Education: Associate degree (ASN, ADN or AAS) from an accredited school of nursing. Work Experience: 1-3 years in the Oncology clinic setting. Licensure: Valid RN license in the State of Florida. Certification: Chemotherapy/Biotherapy Fundamentals of Administration Course completion through the Oncology Nursing Society (ONS) within six (6) months of hire or transfer if the Chemotherapy/Biotherapy Certificate Course completion through ONS/ONCC has not already been obtained. BLS as issued by American Heart Association Skills/Knowledge/Abilities: Oncology background in the clinic setting. PREFERRED QUALIFICATIONS Education: Bachelor’s Degree (BSN) from an accredited school of nursing preferred. Work Experience: 1-3 years in the Oncology clinic setting. Licensure: N/A Certification: Oncology Certified Nurse (OCN) through the Oncology Nursing Certification Corporation (ONCC) preferred. Skills/Knowledge/Abilities: Oncology background in the clinic setting. 1-3 years of experience in Oncology infusion. PHYSICAL REQUIREMENTS Physically agile; may require walking, standing running, bending, stooping, kneeling, climbing or crouching frequently. Medium work that may include lifting or moving objects up to 50 pounds of force occasionally, with or without assistance. May require moving self in different positions to accomplish tasks in various environments including tight and confined spaces, including reaching out and above shoulders or over head. May lift, turn, pull, push, or ambulate people and/or equipment with or without assistance up to 90% of the assigned shift. May be exposed to inside and outside environments with varied temperatures, air quality, lighting and/or low to loud noise. May require working with biohazards such as blood/body fluids and airborne particles following OSHA and CDC Guidelines. Communicating with others to exchange information. Visual acuity and hand-eye coordination to perform tasks. Workspace may vary from open to confined. May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
