In this role, you will support residents with psychiatric needs, collaborate with our behavioral health providers, and ensure high-quality, person-centered mental health care. Key Responsibilities Assess and monitor residents’ mood, behavior, cognition, and psychiatric symptoms Administer psychiatric medications and treatments Report changes and collaborate with psychiatrists, NPs, and counselors Assist with crisis management and de-escalation Support care plan development and documentation in the EHR Educate staff, residents, and families on mental health and coping strategies Participate in GDR meetings and interdisciplinary care conferences Ensure follow-up on psychiatric consults and coordinate referrals Respond to behavioral emergencies following protocol Requirements Active RN license (NY) Experience in psychiatric or mental health nursing (required) LTC/SNF or behavioral health experience preferred Strong communication, assessment, and documentation skills Knowledge of psychopharmacology and behavioral interventions Why Join Us? About the Role: We are seeking an experienced and compassionate Registered Nurse (RN) to join our Home Care team . This role includes both field nursing visits and admissions work , providing quality care and support to patients in the comfort of their own homes. Responsibilities: Conduct patient assessments and complete admission documentation. Develop, implement, and evaluate individualized care plans. Provide skilled nursing care, including medication management, wound care, and patient education. Collaborate with physicians, case managers, and home health aides to ensure coordinated patient care. Maintain accurate and timely clinical documentation in compliance with agency and state regulations. Qualifications: Active New York State RN license (required). Minimum of 1 year of home care or clinical nursing experience preferred. Strong communication, organizational, and clinical assessment skills. Valid driver’s license and reliable transportation.
