General Statement of Duties : The Registered Nurse Inpatient is accountable for patient care consistent with established nursing practices and hospital policies and procedures. The Registered Nurse Inpatient assesses, develops, implements, and evaluates a plan of nursing care for patients commensurate with training and licensure. The Registered Nurse Inpatient collaborates with other members of the healthcare team, receives direction from physicians and other professional personnel, and supervises and delegates to non-licensed staff. Essential Duties : Performs patient assessment and communicates pertinent patient information/data to appropriate care providers in a timely manner. Requires constant mental alertness to observe signs and symptoms of patient conditions and side effects of medications or technical procedures and responds appropriately to ensure optimal patient care. Delivers direct patient care. Completes treatments and procedures as ordered and/or indicated. Provides care related to personal hygiene, activities of daily living, and comfort measures as indicated. Prepares and administers medication and treatments as prescribed by physicians. Documents all patient care activities in accordance with the standard of care as well as changes in the patient’s condition. Supports patient education and discharge planning. Distributes educational material as appropriate. Completes assigned tasks that facilitate discharge planning. Participates in annual and ongoing training, education and development activities to improve knowledge and skills. Requires knowledge of equipment and supplies. General Statement of Duties : All clinical care provided by the Advanced Practice Provider (Nurse Practitioner or Physician Assistant) will be within the scope of the duties assigned by the supervising OrthoIndy Physician. The supervising Physician must be either physically present or immediately available for consultation when services are proved by the Advanced Practice Provider, including care provided using protocols, guidelines, and/or standing order. When the supervising Physician is unavailable, they may designate another Physician who has agreed to temporarily be responsible for the supervision. Essential Duties: Helps plan and implement care of patients, including entering orders into the patient record that are verbal orders or part of standing orders or protocols approved by the supervising physician. Supports supervising physician with both outpatient clinics and surgery visits. Provides direct patient treatment, including but not limited to: performing as a First Surgical Assistant, assists with simple wound care and repair, joint injection and aspiration, administration of local anesthetic and digital blocks, joint and fracture reduction, and application of braces, splints, and other DME and soft goods. Orders radiologic and laboratory tests and studies as necessary to complete history and physical examinations. Assesses patients and assesses normal and abnormal findings obtained from history and physical examination as well as radiology and ancillary studies. Plans and implements care of patients, including; EMR documentation, ordering additional studies: communicates with supervising Physician and additional staff, and prescribing appropriate medications. Helps to counsel individuals and families about their health-related issues. Communicates individualized teaching plans to patients based on their needs. Collaborates with multidisciplinary team members by making appropriate referrals. Demonstrates the knowledge and skills necessary to provide care appropriate to the age and development levels of patients served. The Nursing Technician assists with obtaining vitals, and other taught skills as trained by OrthoIndy. Essential Duties: Obtains and reports vital signs and fluid intake and output. Discontinues IV lines and urinary catheters as directed by the Registered Nurse and documents removal. Observes and reports patient conditions and behaviors to the Registered Nurse. General Statement of Duties : The Case Manager supports patients, families, and payer representatives, medical and allied health team personnel. The position aims to achieve optimal, timely, and durable patient outcomes, customer satisfaction, and management of resources and costs for the patient. The Case Manager serves as a patient advocate to ensure needs and communicated to the appropriate staff. Essential Duties : Makes contact with family or patient representative and performs introductory function and initiates discharge planning. Performs psychosocial assessment to determine pre-morbid history, human supports and available financial and community resources, cultural characteristics and interests, what the patient and family expectations for discharge. In conjunction with the patient, family and treatment team arranges post hospital services such as home care or rehabilitation placement. Promotes compliance with Medicare and payer source requirements. Responsible for contacting payer source to review accuracy of information, providing clinical update and discharge information as indicated by the payer source. Functions as liaison with payer representatives to manage financial resources, including verification of benefits for this and future settings Advocates with the payer on behalf of the patient to obtain needed services. Documents payer source contact, including authorization, contacts, and days approved. Works in conjunction with admissions, billing, medical records, coding, and auditing to facilitate reimbursement. Reviews medical documentation. Ensures completeness and accuracy of information, including appropriate admission orders. Provides patient and family with overview of his/her rehabilitation program, and works in conjunction with the patient and treatment team to determine treatment goals and estimated length of time for completion. Determines available financial resources and directs team planning to ensure conservation of funds and appropriate scope, intensity and duration of service delivery. Works with the physician to determine and maintain appropriate patient status. Responsible for contacting the attending physician to request clarification of documentation or continued stay review questions. Responsible for informing Admissions and other departments of changes in patient status. Reviews status, goals, and outcome barriers with the patient and treatment team and physician, monitors and facilitates progress towards goals. Ensures scope and intensity of services, financial coverage and alternative resources are appropriate to patient needs.
