Transitional/Rehabilitative RN Shift Supervisor - Nights - Full-Time

Job Purpose The Transitional/Rehabilitative RN Shift Supervisor is the first line supervisor on a nursing unit that practices and directs patient care activities during assigned shift. This position provides skilled nursing care in accordance with Nurse Practice Act and requires staff accountability for compliance with regulatory and professional standards for patient care units and staff. Job Duties Duty Percent of Time Supervises nursing staff and the daily duties to perform direct patient care activities. Clinic Nurse Navigator

The Clinic Nurse Navigator serves as a patient advocate, interpreter, educator and counselor for our outpatient clinic patients. Under limited direction, this position functions on the multidisciplinary team to coordinate patient care throughout the continuum of care and serves as a clinical resource for care management. The Wound Care Nurse serves as a clinical expert for wound prevention, assessment, treatment, and skin integrity management. Advanced Practice Provider - Neurology

The purpose for the APP position is to support functions in the neurology program as a clinician, educator, team member, and advocate for our patient population. The APP will see outpatient follow up neurology visits in order to create treatment plans, communicate with patient/family members regarding their care, coordinate with schools (for seizure action plans, IEPs, medication forms and other medical forms), and take first line call for neurology after hours 10 days per month (Monday – Thursday).
