Overview A Nurse Practitioner, certified under section 6910 of the New York State Education Law is a licensed registered nurse who provides primary health care in accordance with written policies approved by nursing/medical administration. Responsible for patient care as outlined within the Collaborative Practice Agreement. NURSE PRACTITIONER TAVR/LVAD/CHF Full-Time 8am – 6:30pm / 4-days per week Responsibilities For direct care providers: Provides age-appropriate care and intervention for ages of patients served according to job description (e.g. assessments, treatment plans, assists with assessments). For non-direct care providers: Provides support services using age appropriate interpersonal communication. Clinical Practice Domain Develops and implements a collaborative practice agreement in the specific clinical area and performs a periodic review of the collaborative practice agreement. Initiates referrals to specialty services and fosters continuity of care. Initiates hospital admission and discharge after collaborative consultation with physician. Works collaboratively with interdisciplinary team to promote optimal patient outcomes. Assessment, treatment and patient care responsibilities are delivered with a knowledge of patient growth and development and are appropriate to the ages of the patients served.
