Job Details ChristianaCare is looking for a full-time Nurse Practitioner for our Silverside OBGYN Practice who has a passion for Women’s Health. The Advanced Practice Clinician (APC) will evaluate, manage, and provide care to women seeking obstetrical and gynecologic care including family planning and prenatal services. Position Overview: The APC will be responsible, but not limited to, the following duties: Provides direct management and ambulatory health care to adolescent, adult and geriatric women seeking OB/GYN services. Responsible for ambulatory health care to women seeking OB/GYN services – women’s health care in the areas of health screening, health education, family planning, screening for sexually transmitted diseases, obstetrical care, and prenatal care of pregnant women. Assess the female patient by performing a comprehensive health history and physical examination. Performs, obtains, and/or orders appropriate laboratory tests to assist in screening and diagnostic evaluation and follow-up. Prescribes all forms of contraceptives including LARC. Monitors progress of prenatal patients and assesses risk status. Perform evaluation of normal and high-risk obstetrical patients. Performs limited OB and GYN ultrasounds. Formulate an assessment and diagnosis. Performs endometrial biopsy. Seeing patient both in-person and virtually. Recommended skills: colposcopy, vulvar biopsy. Assist with discharge planning and perform administrative duties such as completing medical forms, reports, evaluations, studies charting, etc. Monitor the facility’s QI, QM, and survey reports. Assist in the developing plans of action to correct potential or identified problem areas. Assist in calculating the number of direct nursing care personnel on duty each shift. Assign a sufficient number of licensed practical and/or registered nurses for each tour of duty to ensure that quality care is maintained. Assign a sufficient number of certified nursing assistants for each tour of duty to ensure that routing nursing care is provided to meet the daily nursing care needs of each resident. Develop work assignments and schedule duty hours, and/or assist nursing supervisory staff in completing and performing such tasks. Delegate to nursing personnel the administrative authority, responsibility and accountability necessary to perform their assigned duties. Make daily rounds of the nursing service department to ensure that all nursing service personnel are performing their work assignments. Assist the HR Director in developing performance evaluations on an annual basis for the nursing department. Ensure that departmental disciplinary action is administered fairly and proper documentation for such discipline is given to HR Director for personnel file. Assist with infection control monitoring in establishing a TB management program for employees. Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals. Ensure that the department’s policy and procedure manuals identify safety precautions and equipment to use when performing tasks that could result in bodily injury. Assist in identifying, evaluating, and classifying routing and job-related functions to ensure that tasks in which potential exposure to blood/body fluids are properly identified and recorded. Ensure that all personnel performing tasks that involve exposure risks participate in proper in-service training programs. Review nurses’ notes to ensure that they are informative and descriptive of the nursing care being provided, that they reflect the resident’s response to the care, and that such care is provided in accordance with the resident’s wishes. Monitor medication passes and treatment schedules to ensure that medications are being administered as ordered and that treatments are provided as scheduled. Provide direct nursing care as necessary. Coordinate continuing education/in-service training for nursing department. Report suspected or known incidence of fraud to Administrator. Assist the MDS/Care Plan Coordinator in planning, scheduling & revising the MDS, including the implementation of RAPs and Triggers. Encourage the resident and his/her family to participate in the development and review of resident’s plan of care. Maintain confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information. The Med Surg units include: Renal: The Renal/2 South unit is a 24-bed medical/surgical unit specializing in the care of patients with kidney disease, including kidney transplant. The growth in the number of transplants in recent years is a testimony to the expertise of the team of board-certified nephrologists, transplant surgeons and an all-RN nursing staff. A continuum of care is provided for patients undergoing kidney transplant as they are supported through their unique living related, cadaveric or paired exchange donation transplant process. The nursing staff is specifically trained to perform skills unique to this patient population, including peritoneal dialysis through manual as well as cycler methods. The nursing staff actively participates in delivery of specialized care for patients who qualify for cutting-edge pharmacologic research studies under the supervision of nephrologists, transplant surgeons and clinical pharmacologists. An on-site outpatient kidney transplant clinic provides ongoing episodic care to transplant patients. The unit also provides care for a wide variety of general medical/surgical patients with common diagnoses. We have the ability to provide remote telemetry monitoring for 8 beds as needed. Patients complete three hours of therapy daily, including physical, occupational and/or speech therapy. Each patient is followed by a physical medicine and rehabilitation physician. The nursing staff is trained to care for all patient populations and works collaboratively with the therapy staff to achieve our 81 percent discharge-to-home success rate. The unit is CARF accredited and upholds the highest standards of care for the rehabilitation patient population. General Surgery: The General Surgery/5 South unit is a fast-paced, 29-bed unit with all private rooms located on the fifth floor of The Christ Hospital’s south wing. The General Surgery unit cares for adults undergoing complex surgeries, including Whipple’s, DIEP flaps, colon resections, ostomy creations, bariatric weight loss, mastectomies, hernia repairs, head and neck resections, urological procedures, and multiple laparoscopic surgeries. Nurses on the unit are experts in post-surgical care and work closely with physicians, nurse practitioners, case managers, physical therapists, social workers and our wound and ostomy care team. A strong emphasis is placed on the utilization of the Enhanced Recovery Pathway to optimize the surgical patient’s physiologic function and to facilitate their recovery. The general surgery care team is specifically trained to provide care unique to this patient population through our Ostomy Care Associate training. Our team-oriented and caring peer environment is great for both the experienced and new graduate nurse. Joint and Spine/Musculoskeletal: The orthopedic/neurosurgery unit is a fast-paced, 58-bed unit consisting of all private rooms located in The Joint and Spine Center (JSC) on floors 4-5. It has state-of-the art technology including the Navicare Nurse Call system, in room computing, patient information and precaution monitors as well as texting communication. The unit primarily provides specialized care for patients undergoing orthopedic/neurosurgical procedures including total joint replacements, fracture repairs, spinal surgeries, craniotomies and intracranial hemorrhage patients. The unit may also receive medical/surgical overflow. The experience of caring for the diverse patient population on this unit provides staff with a solid clinical foundation in medical surgical nursing. Oncology: The Inpatient Oncology Services/4 West unit is a 27-bed unit. Patients may be admitted to the unit at any time during their journey through the cancer continuum, from initial diagnosis to surgery, chemotherapy, management of complications and terminal care. Patients with medical-surgical conditions and hematological disorders such as sickle cell crisis may also be admitted. Care on this unit is provided by a multidisciplinary team of registered nurses, patient care assistants, physicians, nurse practitioners, case managers, health unit coordinators, an oncology pharmacist, an educator, chaplains, nutritionists, social workers, physical therapists, occupational therapists, and respiratory therapists. Behavioral Health: The Behavioral Services/6 West unit is a 31-bed unit providing a full range of services and comprehensive care to adult and geriatric patients with behavioral health needs. Programs include inpatient and outpatient services. The team consists of psychiatrists, registered nurses, licensed practical nurses, health unit coordinators, social workers, patient care assistants and occupational therapists. Services include diagnostic assessments, treatment team meetings, group and individual therapy, family meetings and partnerships with community providers.
