Nurse Practitioner or Physician Assistant - Outpatient Primary Care

Work is performed in a physician office or offices dedicated to Primary Care, weekend, and on-call coverage on a rotational basis, as required. M-F evenings and Sat 8am-2pm ESSENTIAL RESPONSIBILITIES Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, providing accurate medical record documentation. 25% Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team. Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice. 15% Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and the supervising physician(s).10% Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds and provides wound care10% Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions and recommends community resources as appropriate.10% Triages patient telephone calls, provides monitoring and continuity of care10% Participates, develops, and implements performance and/or quality improvement initiatives. Job Summary: The Preadmission Nurse (PAT RN) has the responsibility for organizing and directing activities in accordance with nursing standards, and applicable federal, state and local guidelines. PAT RN may assume the following PAT responsibilities: contact, by way of telephone, patients prior to surgery to assess medical and medication history, review for appropriate lab, x-ray, EKG orders to determine where and when these procedures are to be completed and provide preadmission teaching. The RN can expect to care for adult patients, including the older adult through geriatric, and pediatric patients from 4 months through adolescence. The RN may perform laboratory testing required prior to surgery as ordered. Job Duties and Responsibilities: Other duties may be assigned: 1. Plans and prepared for the preadmission of the patient. 2. Meets all required deadlines. Demonstrates discretionary ability in establishing priority. 3. Delivers indirect and direct patient care according to established standards, taking into consideration the age and risk factors of the patient. 4. Possess the knowledge and ability to use the nursing process to assess, plan, implement, and evaluate the care of the patient. 5. Uses nursing judgment in decision-making. 6. Acts as a patient advocate in meeting his/her physical and psychological needs. 7. Performs patient assessment and reassessment immediately upon admission and on a regular basis in accordance with standards of care with documentation of these assessments as a basis for planning each patient’s care. 8. Ensures that all nursing interventions and patients' responses are communicated to responsible practitioners as indicated and that all components of the nursing process are documented in accordance with standards of practice. 9. Integrates and coordinates care across settings and among disciplines. 10. Implements physician’s orders effectively and safely in accordance with standards of practice. 11. Ability to recognize and intervene in stressful situations. 12. Maintains BLS, ACLS and PALS certifications as applicable. 13. Maintains or acquires skills on equipment necessary to complete required responsibilities. 14. Documents all records according to established policies and procedures. 15. Assesses information/documents on AdvantX, maintaining patient confidentiality. 16. Assists with reviewing and updating policies and procedures. 17. Performs effectively in stressful situations to fulfill position responsibilities. 18. Provides teaching to patient/parent of child per level of understanding. 19.
