Job Responsibilities and Requirements: PRIMARY RESPONSIBILITIES Demonstrates critical thinking skills in providing care for the perioperative patient by completing a comprehensive preoperative assessment, planning and implementing individualized nursing interventions and evaluating the effectiveness of care and modifying interventions as needed. Prioritizes multiple complex patient and physician needs to delegate and coordinate the care provided by the surgical team to maximize patient outcomes. Coordinates patient care assignments to personnel supervised, considering staff competencies, patient population, equipment used, patient condition, environmental factors, infection control, safety factors and availability of direct/indirect supervision. Communicates effectively with patients, staff and physicians to anticipate needs and develop and implement actions to be taken in response to complaints related to patient care processes. Maintains and enhances professional clinical skills by seeking out learning situations and remaining abreast of current advances in nursing techniques and technologies. Serves as a consultant to all personnel in area(s) of nursing expertise. Teaches and mentors other less-experienced RNs and technicians. Demonstrates advanced clinical judgment as evidenced by knowledge of complex nursing interventions required in multiple surgical specialty areas. Exhibits willingness to take on special projects and initiates actions that work to improve processes within the department. As an SSM Health nurse, I will demonstrate the professional nursing standards defined in the professional practice model. Uses the ANA Code of Ethics for Nurses to guide his/her response to the current and evolving health and nursing needs of our patients and our patient populations. EDUCATION Graduate of accredited school of nursing or education equivalency for licensing EXPERIENCE Two years’ advanced surgical experience required This is an evening shift position - 3-11pm M-F PHYSICAL REQUIREMENTS Constant use of speech to share information through oral communication. Constant standing and walking. Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs. Frequent sitting, reaching and keyboard use/data entry. MDS Registered Nurse or Licensed Practical Nurse

The MDS / Care Plan Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for care and services provided within the Facility. Conducts continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category. Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission. He/she will integrate nursing, dietary, social recreation, restorative, rehabilitation and physician services to ensure appropriate assessment and reimbursement. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assesses and determines the health status and level of care of all new admissions. Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change. Communicates level of care for new resident to all disciplines. Coordinates interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission to facility according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal, state and medical standards. Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay. Responsible for the data entry function to assure accurate data entry and electronic submission of MDS assessments. Verifies electronic submissions of MDS, performs corrections when necessary and maintains appropriate records. Coordinates interdisciplinary participation in completing the MDS for each resident according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal and state standards. Schedules and conducts resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conference. Assists disciplines in formulating and revising care plans. Ensures that resident’s present/potential problems are identified and prioritized; realistic goals are established and nursing intervention is appropriate. Evaluates resident care plans for comprehensiveness and individuality. Assesses the achievement or lack of achievement of desired outcomes. Ensures that resident’s care plan is reassessed and revised appropriately. Responsible for all level of care changes within the facility. Notifies all departments when a level of care change has been made. Generates appropriate forms to complete level of acuity and changes. Transmits forms to the appropriate agency for processing as required by state law. Other duties as assigned. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Location Battle Creek- 300 North Ave - Bronson Battle Creek Hospital Title OB Licensed Practical Nurse ( LPN ) - Bronson Battle Creek Birthplace - OB / Labor Delivery / Mother Baby The Bronson Birthplace in Battle Creek has 15 beds – 11 for labor/delivery/recovery/postpartum (LDRP) care, 4 for antepartum/post-op care, 2 operating rooms and 3 triage bays. Additionally, the unit includes a newborn nursery. Here, pregnant, laboring and postpartum patients, as well as healthy newborns, receive comprehensive care. LPNs on this unit have the opportunity to care for postpartum couplets, assist in operating room procedures and provide support during deliveries. Under the supervision of the Registered Nurse, provides prescribed medical treatment and personal care services which are within the scope of the practice of practical nursing to laboring, ill, injured, convalescent, and disabled persons as delegated, appropriate to the patient’s level of acuity and in accordance with established policies and procedures of the department and organization. Under the supervision of the Registered Nurse, and according to established policies and procedures, the LPN assists with surgical procedures as a scrub member of the obstetrical/surgical team. Prepares instruments and maintains a clean and safe obstetrical/surgical environment. The LPN will assist the Registered Nurse in collecting data, accompanying patients and responding to patient/family requests for services. The LPN may pass medications and administer IV therapy, per medication administration policy. Employees providing direct patient care must demonstrate competencies specific to the population served. Prepares operating room and Labor/delivery room for deliveries and procedures. Assists the Physicians/CNMs/PNPs and nursing personnel before during and after deliveries. Tears down the OR, LDRP room following procedures and deliveries, and assists house-keeping with cleaning as needed. Cleans equipment and returns to appropriate places. Prepares and returns instruments for cleaning and sterilization. Returns unused supplies to appropriate places. Checks, orders and restocks supplies inventories, instruments and equipment in appropriate areas. Rotates stock, stocks patient rooms, supply rooms, procedure carts, medication room, and utility rooms. Picks up and delivers specimens, equipment, meal trays, blood products and stat pharmacy orders. Cleans, cares for, and is able to utilize the computers and Central Fetal Monitoring systems in the obstetrical area. Sorts, files reports. Answers telephones, takes and relays messages, serves a unit communication source.
