Description Patient Care Associate MSH GP9W Medicine w/Stepdown FT Nights EOW As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Documents intake and output and other data as assigned • Obtains patient’s rating on pain scales as a fifth vital sign in all areas except Pediatrics • Performs clinical procedures, such as specimen collection including blood glucose, venipuncture, EKG, application of compression devices, post-mortem care, and maintenance of skin integrity. • Sets up for selected procedures and assists RN’s accordingly. • Assists patient with activities of daily living, such as feeding, bathing and/or toileting. Assists the patient in room maintenance, such as bed-stripping and trash removed, when necessary • Performs on-to-one observation or close observation status of patients, e.g. safety, elopement risk, or suicide prevention. • Maintains “isolation status” for patients under direction of an RN and according to set procedures • Performs scheduled hourly care checks/ rounding on patients with emphasis on caring for toileting needs, comfort, and positioning and pain awareness. Summary Job Description: Job Description Registered Nurse : Cardiovascular, Thoracic Telemetry unit. Unit Overview: 4G, Cardiovascular, Thoracic Telemetry unit. 4G is a 20 bed telemetry unit. This unit specializes in post-op CABG patients, post-op cardiac surgery, CHF and moderate risk chest pain patients. The population has multiple co-morbidities and complex care. The patient population is transferred from ICU, ED and PACU. Shift : Full-time, Nights Job Description: The Registered Nurse (RN) implements professional nursing practice centered on the patient as a whole person and the creation of a caring environment focused on healing and wellness. The Registered Nurse (RN) is an exemplar for compassion, acceptance, respect, and empathy honoring nursing as a distinct discipline and supporting an environment focused on healing and wellness. As a clinical nurse, this individual spends majority of his/her time (> 50%) coordinating, ensuring and providing direct patient care activities on the unit through: provision of nursing rounds, assessments and treatments; facilitating admission, transfer and discharge activities; provision of patient and family-centered education; care coordination and individualized treatment planning as applicable, etc. Minimum Education, Experience, Training and Licensures Required: Current RN licensure or work permit in Kentucky. Bachelor's or an associate degree in nursing from an accredited college/university. Those with an associate degree in nursing and less than 10 years of experience must enroll within 12 months of hire in a bachelor’s degree program and must have achieved the degree within 5 years. Basic life support- Applicants will have 30 days from their hired date to provide completion of this certification if they don’t have it already.
