Summary Functions as a clinical expert and care coordinator for a specific patient population interfacing with administrative, clinical nursing staff, physicians and patients/families. The Adolescent and Young Adult (AYA) Nurse Navigator functions as a clinical expert and care coordinator for AYA Program of the Holden Comprehensive Cancer Center (HCCC). The AYA Nurse Navigator functions as a clinical liaison between the primary healthcare team and patients/families with attention to the unique experiences to AYA cancer patients including psychosocial care, onco-fertility and clinical trial enrollments. They will provide seamless patient-centered navigation through the continuum of care from initial diagnosis until transition into survivorship or end-of-life care Position Responsibilities Serves as a clinical expert and resource to AYA patients with a cancer diagnosis ages 18 to 39. Meets with patient at enrollment to AYA program to make a face-to-face introduction and provide navigation related education. Places follow-up orders for routine check-ins with AYA patients throughout their cancer trajectory to educate and access for unique needs.
