If a transfer requires two caregivers or cannot be completed safely, the caregiver must not attempt it alone. The caregiver must keep the client safe and contact the Helping Hearts office immediately for assistance and further direction. Proactive Fall Prevention and Safety Monitoring Because this client is at significant risk for falls, the caregiver must remain alert, attentive, engaged, and safety-focused throughout the entire shift. Meaningful companionship is an important part of this assignment. Concerns must be reported immediately. Caregivers must not wait until the end of the shift to report an urgent change, fall, near fall, injury, or safety concern. Small Dog in the Home The client has a small dog. Clinical Care * The Physician Assistant (PA) or the Nurse Practitioner (NP) provides assessment, triage, and therapeutic management of patient/family needs throughout the disease process as an integral part of an interdisciplinary team.
