Responsibilities: • Conduct comprehensive primary care assessments and develop individualized care plans for adult and geriatric patients. • Manage acute and chronic medical conditions common in aging populations, including hypertension, diabetes, COPD, heart failure, and cognitive decline. • Provide medication management, prescription refills, and ongoing monitoring for complex medical regimens. • Perform routine and preventive care visits, including wellness exams, screenings, and immunizations. • Conduct rounds at assisted living facilities and provide follow-up care in the outpatient clinic setting. • Collaborate with behavioral health clinicians, social workers, and facility staff to ensure coordinated, whole-person care. • Communicate effectively with patients, families, and caregivers regarding diagnosis, treatment options, and care transitions. • Maintain accurate, timely, and thorough documentation in EHR systems. • Adhere to all regulatory requirements applicable to ALF and outpatient clinical environments.
