Responsibilities Job Summary As a member of a multidisciplinary medical team, the Nurse Practitioner will work collaboratively to deliver high quality, direct clinical care. Duties and Responsibilities: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Performs evaluations, H&Ps, detox management, and MAT as per SOP H&Ps within 24 hours of admission/provider/month (Medical) Psych Evals within 48 hours of admission/provider/month (Psychiatry) Orders labs and other clinically appropriate testing and prescribes medications and treatments Participates in the development and implementation of treatment plans Consult with other physicians/clinicians about each patient’s specific need and refers to specialists as necessary Documents all patient encounters and diagnostic test results accurately and timely in the Electronic Medical Record (EMR) Participates as an active member of the clinical team Attends monthly medical staff meetings with pertinent clinical staff and UR Other duties as assigned including but not limited to participate on clinical/medical committees, providing education and quality review. Job Descriptions : • Deliver a wide range of infusion therapies in the comfort of patients’ homes, including antibiotics, biologics, IV fluids, TPN/PPN, IVIG, blood products, and specialty medications. • Perform PICC line and port care, dressing changes, and the access/de-access of implanted ports and Broviac lines. • Conduct patient assessments, document findings, and collaborate with the infusion pharmacy, physicians, and internal clinical teams to ensure continuity of care. • Educate patients and caregivers on therapy administration, side-effect monitoring, and safety protocols. • Maintain accurate, timely documentation using electronic systems (EMR). • Participate in ongoing clinical training and skills validation as needed. Scheduled Hours 40 Position Summary Position functions as a Licensed Practical Nurse (LPN) in the department, and may be strictly administrative, clinical or a combination of both. An LPN in an administrative role acts as a liaison between physicians and patients, fielding phone calls, referring physicians, clinical justification for insurance reimbursement of patient medication or procedures, communicates lab results and patient education. They document patient information in Epic. May be asked to cover clinical care in the event of a call in. An LPN in a clinical role prepares and maintains exam rooms, interacts directly with patients in the office completing tasks such as vital signs, EKGs, phlebotomy and administration of medication vaccines, and allergy shots under the order of a physician or practitioner. These tasks vary according to the department specialty which dictates the skills required. This LPN would assist with setting up for procedures, assist with procedures and directs patient flow. They document patient information in Epic. Provides nursing care, under supervision for patients according to established standards and practices. May be asked to cover administrative coverage in the event of a call in. Aids in the updating and data entry in electronic medical records. Promptly alerts physicians of abnormal test results. Assists in the assessment, monitoring, and termination of prescribed tests. Observes, monitors, and responds appropriately to changes in patient treatments. Participates in development, implementation, and evaluation of patient care plans with other members of the patient care team. Answers phone, screens and routes calls providing information in accordance with Division policy. Provides assistance in the day-to-day administration of physician practices.
