Licensed Practical Nurse (LPN) - Emergency Department - St. Charles Hospital Job Summary: The LPN (Licensed Practical Nurse) delivers safe and efficient care to patients using nursing interventions. The LPN is responsible for general clinical and clerical duties associated with direct and indirect patient care. Essential Functions: Gives direct nursing care and assists with care of the patient at the direction of a Registered Nurse, Licensed Physician, Physician Assistant, or Advanced Practice Nurse Implements the plan of care by providing direct basic nursing care in accordance with the LPN scope of practice Performs phlebotomy services Contributes to the plan of care with input from patient/family and other members of the healthcare team Administers ordered medications and treatments, within scope of care, prescribed by a Licensed Physician, Physician Assistant, or Nurse Practitioner while using appropriate medical equipment as needed Other duties as assigned Education: High School Degree or GED Completion of an accredited LPN (Licensed Practical Nursing) Program Certifications: Current state licensure as an LPN or LPN applicant (Licensed Practical Nurse) Basic Life Support (BLS) – American Heart Association (preferred, not required) Experience: One year of clinical patient care experience (e.g., PCT, CNA, LPN) in an acute care setting, long-term care facility, or physician practice (preferred, not required) Training: EPIC Electronic Health Record (EHR) training (preferred, not required) Completion of Basic Arrhythmia class with a passing grade on exam (required for Telemetry/Remote Telemetry units) Mercy Health is an equal opportunity employer. You are a key part of the care team - delivering treatments, monitoring patient conditions, and reinforcing education to support successful outcomes. What You’ll Do Provide direct patient care in accordance with the state Nurse Practice Act Implement and support the plan of care developed by the RN Deliver skilled nursing services, including treatments, medication support, and wound care Monitor patient condition and report changes to the RN and care team Educate patients and caregivers on self-care, treatments, and disease management Maintain infection control and proper use of equipment and supplies Document patient care accurately and timely within the EMR Communicate effectively with physicians, nurses, and interdisciplinary team members Participate in case conferences, team meetings, and ongoing training Support coordinated care by maintaining consistent communication across the care team What You Bring Active LPN/LVN license in the state of practice Graduate of an accredited practical/vocational nursing program Current CPR certification Minimum one (1) year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus) Strong clinical and observational skills Clear communication and patient education abilities Ability to follow care plans while recognizing and reporting changes in condition Basic proficiency with EMR systems Valid driver’s license and reliable transportation What Sets You Apart Experience in home health or community-based care Strong patient engagement and relationship-building skills Ability to work independently while staying connected to a team Familiarity with Homecare Homebase (HCHB) What to Expect Field-based role providing care in patients’ homes within an assigned territory Close collaboration with an RN and interdisciplinary team A structured yet flexible daily schedule A role focused on delivering consistent, high-quality patient care Performance expectations tied to quality, documentation, and patient experience If you’re passionate about delivering compassionate, hands-on care and want to be part of a team that truly values the work you do - VitalCaring is the place for you.
