Description The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. 3. Communicates effectively and professionally with patients, family and all members of the Interdisciplinary Patient Care Team. 8. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively. Patient Experience 1. Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions including, but not limited to, Admission Welcome, Bedside shift Report, HELP (High Risk Medications, Equipment, Lines, Drains, Airway, Pain and Plan of Care), AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. 2. Considers the patients values, preferences, expressed needs and knowledge in all aspects of care. 3. Establishes and maintains a therapeutic relationship with the patient and family. 4. Uses evidence-based practices to increase understanding of patient's perceptions of care. 5. Initiates service recovery for patients and family members who have concerns and escalates to Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator or Patient Relations as needed. 6. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. 7. Supports staff use of evidence-based practices to increase understanding of patient's perceptions of care. 8. Conducts Purposeful Hourly Rounding (PHR) on assigned patients addressing the 4 Ps. Quality and Safety 1. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. 2. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. 3. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. 4. Engages in formal and informal peer and institutional review processes. 5. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. 6. Contributes to quality and safety practices and required compliance measures. 7. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients. 8. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. 9. Utilizes institutional information technology by documenting nursing practice to support quality and performance improvement initiatives. Operations 1. Performs charge nurse duties as assigned. 2. Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. 6. Facilitates patient throughput on a shift basis. Description The Nurse Manager (NM) is responsible for the management of nursing practice and operations on a designated unit/service setting such as inpatient, procedural, emergency department or ambulatory care. This includes, but is not limited to, the management of: clinical nursing practice, staff recruitment, retention, labor relations, supply chain and budget, staffing, employee performance evaluation, quality improvement, staff education, medical records, internal and external regulatory and survey requirements. The Nurse Manager collaborates with multiple professionals to support and coordinate the provision / management of patient care. This individual works closely with the Nursing Clinical Director, unit Dyad partner and staff in insuring that the Service/Unit setting maintains the highest standards of patient care and is responsive to the needs of the patient & family as well as the organization. Responsibilities Patient Care Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. Reviews unit progress, changes, and compliance with quality and safety metrics with clinical nurses and other members of the healthcare team. Responds to escalations of potential safety hazards or gaps from best practice by correcting concerns following institutional chain of command protocol. Monitors institutional information technology infrastructure and corrects gaps in clinical nursing documentation. Leads staff participation in quality and safety initiatives and mandated compliance measures. Demonstrates accountability for all unit quality and safety practices and compliance, and ensures required evaluation of staff competencies is maintained. Operations and administration Supports implementation of the mission, vision, goals and objectives of the department, throughout the clinical service and individual patient care units. Provides administrative and clinical oversight to ensure that all activity (staffing, support services, inter disciplinary collaboration, resources and financial viability) supports the goals of the organization. Works collaboratively with physician dyads and all members of the healthcare team to promote patient care and regulatory standards. Prioritizes recruitment and retention strategies in accordance with system and local hospital retention goals. Maintains a nursing unit in compliance with all regulatory agency mandates, including the regulations of NYS DOH and The Joint Commission. Oversees 24 hour patient care delivery, throughput, staffing and processes to ensure efficient and effective operations in all clinical specialties. Implements and monitors unit salary and supply budgets, including capital equipment allocation, budget variances, management of OT and vacancies according to MSHS financial goals. Monitors availability and maintenance of nursing equipment and supplies Maintains position control of established positions and manages the recruitment and retention of qualified staff. Maintains patient/employee confidentiality in the management of information. Evaluates nursing staff performance and mentors, coaches, counsels and disciplines staff as needed. Evaluates all newly hired/transferred staff and participates in the decision about passing the probationary period. Participates in the ongoing and completes the annual performance appraisals for all staff. Completes performance improvement plans of employees as needed and shares feedback with the Associate Directors/Directors and Senior Directors accordingly. Interprets and implements the associated collective bargaining agreements and System and Hospital policies and procedures. Performs disciplinary actions as necessary with collaboration with Nursing and Labor Relations. Acts as a management witness in disciplinary activities. Insures all staff have met their orientation goals/weekly progress reports and annual competency requirements and participates in the Competency Planning Tool for unit/service. Conducts regular staff meetings to ensure communication with staff about progress, changes and compliance with all hospital policies and procedures. Maintains records of minutes for staff team meetings. Encourages, recognizes and rewards staff participation in quality and safety initiatives, compliance and successes. Facilitates organizational change initiatives by using effective strategies. Leads a shared decision making model by establishing a structure of shared governance, just culture and staff engagement in practice councils, departmental meetings and initiatives. Responds to protest of assignment in a timely manner as detailed in the collective bargaining agreement and discusses the resolution in unit staff meetings.
