Communicate with physicians / charge nurse / co - workers, as appropriate regarding changes in the patient's treatment, clinical condition including results of diagnostic studies. Has the ability to respond quickly and accurately to changes in condition or response to treatment. Qualifications Education and Work Experience Must have current and valid license as a Registered Nurse issued by the state. BCLS (AHA) upon hire and maintain current. Previous Acute Care hospital experience preferred. Previous Wound care knowledge and or experience preferred (1 Year). Wound Care Certification within first year of hire. Job Summary Responsible for overseeing activities within the team in collaboration with Manager/Director. Provides care to medically complex patients in collaboration with the physician in a specialized acute care setting. Works autonomously and has authority, responsibility, and accountability for assessment, planning, implementation and evaluation of wound, ostomy, and continence care program throughout the inpatient care areas. Serves as a resource for wound care management, including identification, assessment, treatment, and prevention. Provides specialized consultation on wound, ostomy, and continence issues for patients, families, nursing and medical staff. Serves as a consultant in the management of patients with draining wounds, fistula and tubes. Serves as a consultant for the treatment of patients with altered skin integrity due to pressure necrosis, drainage or incontinence. Works with physicians and nursing staff to provide updated information regarding wound care. Conducts rounds on all patients with wound/pressure ulcers on a daily and as needed basis.
