Full-time Mon-Fri. 8a-4pm $93,000-$103,000 Position Summary: The Interventional Care (I-Care)/Wound Care is responsible for overseeing and auditing nursing and restorative care tasks, ensuring compliance with documentation, orders, and care plans. This role involves providing direct care in areas such as intravenous therapy, respiratory therapy, and restorative rehab, while also supporting fellow nurses performing related therapies or treatments. Responsibilities Include: Resident Assessment: Upon admission, readmission, and reevaluation, assess each resident’s respiratory status, risk factors, and needs relevant to nursing, rehabilitation, and intravenous treatment, in collaboration with an RN and medical professional. Initiate Orders and Care Plans: Initiate all appropriate orders, tasks, consults, and care plans for respiratory, nursing, restorative, and intravenous therapies as necessary. Review Orders and Documentation: Regularly review current orders, tasks, care plans, and documentation to ensure they are appropriate and up-to-date. Ongoing Documentation: Document the rationale for current treatments and their continuation in the resident’s medical record as needed; Conduct and document pre- and post-assessments for all treatments daily, ensuring continuity of assessments. Monthly Administration Record Review: Review all administration records monthly, and at the time of treatment discontinuation, to ensure there are no missing signatures or errors. Interdisciplinary Team Collaboration: Attend all relevant meetings to discuss residents with acute changes, active treatments, or those showing declines that may benefit from a nursing restorative/maintenance program. Qualifications Include: NY RN license required with a minimum of one year infusion experience Organized manner to maintain schedule and generate reports in a timely fashion. Overview The Wound Care Services Manager is responsible for the delivery of wound-related patient care that promotes the safety and well-being of all patients. Plans, directs, coordinates, and evaluates the overall clinical practice including fiscal, material, and human resource management. Participates in the development of policies and procedures for the department. The Manager is actively involved in department activities, transdisciplinary team activities, and activities to ensure individualized, patient-centered health care for all patient populations admitted to OBH Brookdale University Hospital. Responsibilities Works with the wound care physicians to provide a comprehensive treatment program for patients, addresses patient issues, and works through all patient/employee-related matters. Educates referring providers to purpose, function, and realistic expectations of the Clinic. Educates nursing personnel on wound care techniques and skills. Coordinates and revises present wound and/or HBO services by planning, developing, and implementing new service programming compatible hospital goals. Develops appropriate protocols and objectives for programs exhibiting knowledge of the programs by communicating needed information and/or referring requests for information to appropriate sources. Provides leadership in the development of performance standards for employees. Evaluates and counsels’ employees on their performance. Plans and develops educational in-services for the department. The knowledge and skills necessary to perform the position requirements are demonstrated through the successful completion of competencies established for the position, to include population served, and other special needs of patients or customers served by the department. Adheres to OBH Brookdale University policies, procedures, all safety plans, and all standards imposed by regulatory organizations. Delivers direct patient care appropriate to the patients served. The ages of the patients served are adult (18-65 years of age) and geriatric (65 and older) populations. Regular attendance and timeliness are required. Qualifications Education: Graduate of accredited Nursing school BSN. Master’s Degree in Nursing preferred.
