JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of chapters to the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of RCW 74.34.020 or 26.44.030 to the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. JOB LIMITATIONS The hospice aide will not function in any manner viewed as the practice of nursing according to the Nurse Practice Act. Specifically, the hospice aide will not administer medications, take physician's orders or perform procedures requiring the training, knowledge and skill of a licensed nurse, such as sterile techniques. POSITION QUALIFICATIONS 1. Completion of a state approved certified nursing assistant course or equivalent training 2. Nursing Assistant Certified (NAC) in the state of Florida 3. Current Healthcare Provider CPR (Cardiopulmonary Resuscitation) certification ((may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer) 4. Current Food Handlers Permit 5. High school diploma or General Equivalency Degree (GED) 6. Ability to read and follow written instructions and document care given. 7. Understands hospice philosophy, comfortable providing specialized care to the terminally ill. 8. Prior experience in hospice, orderly, acute , rehabilitative, or long term care facility. 1 year previous home health/hospice experience preferred 9. Basic computer skills to document in electronic medical records (EMR) 10. At least 18 years of age. 11. Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order. 12. Bilingual Preferred (English, Spanish or Creole).

Reviews and reports the results of the health status assessment and physical examination, pertinent laboratory data, radiographic data and psychosocial assessment of assigned patients to the provider and takes appropriate action based on standard or verbal orders. Assesses patient/family needs and provides teaching and other instructional material. Provides psychosocial support to patients and families. Maintains patient confidentiality. Prepares and administers therapies under the direction and supervision of a physician. Administers all medications/blood products using correct medication/blood product, dosage/amount, route, time and technique. Assures pre-authorization and appropriate consent forms are obtained prior to administration of therapies where applicable. Facilitates dose modification by assessing patient, assessing labs, consulting protocol or treatment plan, calculating dose, and consulting physician regarding dosage modifications, problems and symptom management Performs venipuncture, accesses implanted ports and maintains central lines catheter devices. Maintains accurate documentation of assigned patient care and treatments provided. Applies knowledge of reimbursement procedures and charge capture. Maintains work environment Restocks the supply area as needed. Follows OSHA guidelines when handling cytotoxic agents, biohazardous waste and any other material potentially infected with a blood borne pathogen. Maintains all disinfection protocols. Maintains equipment according to quality control standards. Facilitates clinical operations and patient care Manages patient medication refill requests from the pharmacy telephone line and records the information in the patient chart. Assists physician with exams and procedures. Maintains efficient and appropriate patient flow in and out of exam rooms. Facilitates and schedules consultations, pre-testing, diagnostic tests or procedures, referrals and future appointments for patients. Triages patient phone calls as needed and documents relevant information in the patient's chart. As necessary, facilitates communication between patient/family and interdisciplinary team and coordinates services related to patient care. Assist patients in locating available community resources. Programs portable infusion pumps for continuous drug delivery and provides instruction to the patient and/or caregiver in the use of the pump at home. Demonstrates knowledge of ambulatory infusion pump use and maintenance, cleaning pumps after use, and proper documentation of pump usage. Attends required, applicable meetings. Keeps current licensure and requirements for the same including continuing medical education. Remains current on all compliance requirements and in part, demonstrates knowledge of Adheres to confidentiality, state, federal, and HIPPA (Health Insurance Portability and Accountability Act) laws and guidelines with regards to patient’s records and information. Manages symptoms/toxicities/health abnormalities related to the intense high dose therapy, stem cell rescue and rehabilitation following rescue in collaboration with transplant physician and/ supervisor. Reports data to company for Transplant related registries and appropriate bodies, International Autologous and Blood and Marrow Transplant Registries. Incorporates company Transplant Network recommendations into clinical practice as assigned. * Specialty Specific Responsibilities For Primary Nurses assigned to a Physician, Additional Responsibilities Include: Manages medication refill requests from pharmacies, documenting details in patient charts. Advocate for patients. Support their empowerment in the care process. Facilitates referrals as part of the care plan, treatment course, or advance care planning, in collaboration with scheduling teams, and as directed by a provider. Acts as the patient liaison between healthcare providers and patients. Assist patients in accessing community resources as needed. Assumes additional duties as assigned, including infusion room nursing coverage. Travel : This position is expected to provide support across Cancer Center of Kansas Wichita locations as assigned. Supervisory Responsibilities: None. Qualifications The ideal candidate for the position will have the following qualifications: Education and or Experience: Graduate from an accredited program for professional nursing education, BSN preferred. RN state license and current registration with the State Board of Nursing in practicing state. Current Basic Life Support (BLS) certification. Maintains working files of reported cases, suspects and individuals with latent tuberculosis infection (LTBI). Monitor treatment regimen, including ongoing assessment, response to treatment, HIV status, positive and negative motivational factors influencing adherence and any unmet educational needs of the patient. Ensure contact investigation is completed, and provide education about TB infection and disease. Communicate current communicable disease status through monthly report summaries. Evaluate outcomes of direct patient care, educational programs, and research. Referral of clients needing medical evaluation and/or treatment for LTBI or active TB to primary care provider or TARC clinic. Facilitates TARC and Refugee Clinics as needed. Includes inventory management, scheduling, referrals, and clinic management. Participate in community meetings, health forums, and board meetings to review community services available, and share services available through MCPHD TB Program. Provide assistance to the TB Medical Director, TB Program Coordinator, TB Staff Physicians, Community-Based Care, Immunizations, Infectious Disease, and Refugee Health programs. DHO/PHN orientation and training. Provides technical assistance to Public Health Nurses (PHNs) case managing TB clients. Recommend TB skin test certification for nurses in school or office settings. TB education to external providers. Evaluate effectiveness of community interventions and individual case management when indicated. Participate in TB case finding. Periodically review and update policies and procedures for the Tuberculosis Control Program, and the Communicable Disease Rule. Plans and implements annual homeless shelter fairs. Qualifications Completion of a Bachelor’s degree in nursing from a National League for Nursing accredited program with emphasis on epidemiology, infectious diseases, microbiology, statistics, or related courses. Master’s degree in public health nursing, public health, or epidemiology encouraged. 3 years of experience as a professional nurse, including at least one year of hospital practice and 2 years of practice in a public health setting with case (progressive) management responsibilities. Experience working with refugee and immigrant populations. Experience in creating and generating data sets and reports for epidemiological use preferred. Experience working in TB or infectious disease clinic preferred. Licenses/Certifications Required Current Indiana license as a Registered Nurse. Valid Indiana driver’s license. Now Hiring – RN On Call Nurse (Weekdays) The RN-On Call Nurse is responsible for, but not limited to: Assumes responsibility for a patient visit or on call schedule which includes the assessing, planning, implementing and evaluating phases of the nursing process Provides hospice nursing care based on systematic assessment docusing on pain control and symptom management Initiates communication with attending physicians, other hospice staff members and other agencies as needed to coordinate care and use of resources for the patient/family Maintains regular communication with direct supervisor to review assignments Maintains up to date documentation at point of service so that problems, plans, actions and goals are accurately and clearly stated and charges are reflected as they occur in accordance with agency policy and Medicare regulation Administers medication in accordance with all federal and state laws and the policies of the Hospice along with LTC staff Maintains current patient records and documents in accordance with the policies of Ascend Hospice & Palliative Care MA to ensure that problems, intervention, goals and outcomes are accurately and clearly states and charges are reflected as they occur Assess and collaborates with Medical Director to determine initial and on going eligibility of the hospice patient Completes an initial assessment of patient and family to determine home care needs Can anticipate end stage disease process and related pain and symptom management Regularly re-evaluates patient's nursing needs Uses assessment data to determine care plan needs Initiates appropriate preventive and palliative nursing procedures Counsels the patient and family in meeting end of life needs Performs a comprehensive death note when appropriate Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition Communicates with other members of the IDT to coordinate the care plan Participates in On-call duties as defined by the on-call policy Ensures that arrangements for equipment and other necessary items and services are needed Ascend Hospice & Palliative Care MA - is part of the Ascend family of Healthcare Companies.
