Objective The MDS Coordinator assists the Director of Nursing and the RN Assessment Coordinator with ensuring that documentation in the center meets Federal, State, and Certification guidelines. The MDS Coordinator coordinates the RAI process assuring the timeliness, and completeness of the MDS, CAAs, and Interdisciplinary Care Plan. Principal Responsibilities Assists the center in assuring adherence to Federal and State regulations and certification. Actively participates in the regulatory or certification survey process and the correction of deficiencies Reports trends from completed audits to the Quality Assurance Committee Assures the completion of the RAI Process from the MDS through the interdisciplinary completion of the plan of care. Initiates and monitors RAI process tracking, discharge/reentry and Medicaid tracking forms through the PointClickCare system. Follows up with staff when necessary to assure compliance to standards of documentation. Completes patient assessments, data collection, and interviews staff as necessary to assure good standard of practice and as instructed in the current version of MDS User’s Manual. Facilitates accurate determination of the Assessment Reference Date that accurately reflects the patient’s care needs and captures all resources utilized to ensure appropriate payment by Medicare/Medicaid and insurance programs. Ensures timely submission of the MDSs to the State with proper follow-up on validation errors. Maintains validation records from the submission process in a systematic and orderly fashion. Qualifications Graduate of an approved Registered Nurse / License Vocational Nurse program and licensed in the state of practice required. Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. Excellent knowledge of Case- Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required. Through understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set. Knowledge of the care planning process.

Report missing ID bands to admission coordinator immediately. • Make beds, clean bedside and overbed tables. • Serve drinking water as required. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Pass water pitcher as per established policy and procedure. • Dress residents neatly and in their own clothing. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Certified Nursing Assistant (CNA) CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30° for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Benefits and Perks: · Health Insurance · Dental + Vison · 401k Essential Duties & Responsibilities: • Meet physical and sensory requirements stated below, and be able to work in the described environment. • Identify and participate in process improvement initiatives that improve the customer experience, enhance work flow, and/or improve the work environment. • Lead or participate in Daily PPS meetings, weekly Medicare meetings, and month end meetings to assure federal billing requirements are met. • Oversee the coordination and participate in the completion of the Resident Assessment Instrument (MDS, CAA’s and Care Plan) in accordance with current Federal and State Regulations. • Assist in completion of the Resident Assessment Instrument with the Interdisciplinary Team. • Notify all Interdisciplinary Team members of the MDS Assessment schedule for all payer sources. • Notify all Interdisciplinary Team members of changes to the MDS Assessment schedule for both all payer sources. • Information to complete the MDS is to be collected using the medical record, bedside assessment, and staff, resident and/or family interviews. • Develop and monitor a system to verify that all Interdisciplinary Team members have completed, dated, and signed the assessments according the Federal Regulations. • If a member of the Interdisciplinary Team is absent the MDS Coordinator is to verify that the all sections of the MDS are complete, accurate, and attest to the accuracy of the sections. • Review prior to closing and transmitting the MDS to assure all sections have been completed, dated, and signed according to Federal Regulation. • Verify all MDS documents required either by Federal and/or State Guidelines or Organizational policy are easily available at all times. • Validate the ARD and Reason for Assessment with Therapy Services prior to transmission. • Assure MDS assessments are transmitted timely as set forth by the Federal Regulations. • Review the Final Validation Note fatal records that were rejected and take appropriate action assuring a rejected MDS is accepted in the Federal Repository prior to end of month billing. • Upon completion of the MDS the Care Area Assessments (CAA’s) must be completed timely according to Federal Regulations. • The CAA’s must contain sufficient information that supports whether or not the Care Area triggered will be Care Planned. • The CAA will clearly state whether the problem “will proceed to care plan” or “will not proceed to care plan.” • When it is determined that a triggered Care Area will not be care planned the reasoning for not proceeding must clearly indicate clinical rational. • Consult the CAA Summary Sheet and verify that all triggered CAA’s and corresponding narrative summaries have been completed, dated, and signed by the appropriate disciplines. • Using information from the MDS Assessment and CAA’s develop an Interdisciplinary Plan of Care. • Review Plan of Care at least quarterly and with each Comprehensive Assessment to assure changes during the quarter are included and updated. • Care Plans must contain problem, goals, and interventions. • Lead or Participate in Interdisciplinary Care Plan meetings. • Provide ongoing education of the RAI process to all interdisciplinary team members, direct and indirect caregivers, business office, etc. with all changes and as appropriate. • Other special projects and duties, as assigned. Job Requirements: • Registered Nurse/Licensed Practical Nurse with required current state licensure. • Minimum three (3) years of clinical experience in a health care setting; long-term care setting preferred. • Must have a current/active CPR certification. • Personal integrity and professionalism to work effectively with the Interdisciplinary Team, Patients, and Families. • Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care. • Self-motivated to learn and master the RAI Process in an effort to attain or maintain the patient’s highest practical level of function. • Excellent analytical and deductive reasoning skills. • Effective verbal and written English communication skills. • Demonstrated basic to intermediate skills in Microsoft Word, Excel, Power Point and Outlook, Internet and Intranet navigation. • Highest level of professionalism with the ability to maintain confidentiality. • Ability to communicate at all levels of organization and work well within a team environment in support of company objectives. • Customer service oriented with the ability to work well under pressure. • Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity. • Strong analytical and problem solving skills. • Ability to work with minimal supervision, take initiative and make independent decisions. • Ability to deal with new tasks without the benefit of written procedures. • Approachable, flexible and adaptable to change. • Function independently, and have flexibility, personal integrity, and the ability to work effectively with employees and vendors.
