Job description:

Our Assisted Living Community Amber Court of Pelham Gardens, located in a beautiful residential area in the Bronx, is seeking a Registered Nurse for the Position of Wellness Director to join an amazing team.

Responsibilities:

  • Pre-Assessment and assessment of new admissions & re-admissions to the adult care facility

  • Requires oversight of case management activities; case manager of record.

  • Work with the health care team to assure that our residents receive the best quality care

  • Implement policies and procedures related to the wellness of the residents, including infection control & prevention

  • Perform and timely submit UAS Assessments via the Health Commerce System

  • Organize resident care information and participate in Quality Improvement and other quality initiatives

  • Provide ongoing guidance to the care team on Wellness processes, regulations and initiatives

  • Perform duties in accordance with Assisted Living NYS DOH guidelines

  • Perform duties in accordance with licensure as a Registered Nurse in New York.

Requirements

  • Candidate must be a Licensed Registered Nurse (RN) currently registered in New York State.

  • Candidate must have the actual experience to Submit UAS Uniform Assessments via the Health Commerce System

  • Candidate should have a warm and compassionate nature, able to interact with the geriatric population and staff in an empathetic manner

  • Able to work as part of a team and contribute to the success of the company

  • Prior experience in Assisted Living, Skilled Nursing or a related senior focused environment

  • Prior management experience preferred

  • Candidate must have completed UAS training

  • Excellent computer skills needed.

This position reports to the Regional Director of Nursing and the facility Executive Director
