Job Summary – Nurse Practitioner (Wound Care)

Dr. MobiCare is seeking a licensed Nurse Practitioner with experience or interest in wound care to join our clinical team. This role focuses on delivering high-quality, patient-centered care while collaborating with a multidisciplinary team to support healing and overall wellness.

Key Responsibilities

  • Perform comprehensive physical exams and wound assessments for patients with acute and chronic conditions

  • Evaluate wound-related conditions and comorbidities to support optimal healing outcomes

  • Perform wound debridement and dressing changes in accordance with clinical best practices

  • Develop and implement individualized care plans in collaboration with the Medical Director and care team

  • Accurately document patient encounters, procedures, and communications in the EMR within 24 hours

  • Educate patients and caregivers on wound care, prevention strategies, and chronic condition management

  • Communicate effectively with primary care providers, specialists, and interdisciplinary teams

  • Collaborate with clinical staff and leadership to maintain efficient workflows

  • Participate in clinical meetings, training, and quality improvement initiatives

  • Advocate for patients by ensuring access to appropriate care and follow-up services

  • Maintain compliance with clinical, regulatory, and safety standards

  • Stay current with CMS guidelines, documentation requirements, and best practices

  • Maintain licensure and pursue ongoing education in wound care and related specialties

  • Perform additional duties as assigned

Qualifications

  • Active, unrestricted Nurse Practitioner license in applicable state(s)

  • Clinical experience in wound care or a related specialty preferred but will train

  • Strong skills in debridement, and care planning

  • Excellent communication and documentation skills

  • Ability to work independently and collaboratively within a clinical team
